Pharmacology

Andepram 10 mg works as a selective inhibitor of serotonin reuptake. By preventing serotonin from being reabsorbed into neurons, it increases serotonin availability in the synaptic cleft, which improves mood and reduces anxiety.

The drug has low or negligible affinity for other receptors, including:

• 5-HT1A, 5-HT2

• Dopamine D1 and D2 receptors

• α1-, α2-, β-adrenoceptors

• Histamine H1, muscarinic cholinergic

• Benzodiazepine and opioid receptors

This receptor selectivity contributes to its targeted action and generally favorable side effect profile.

Dosage and Administration

Adults:

• Initial dose: 10 mg once daily

• If required, may increase to 20 mg after a minimum of one week

• Panic disorder: Start at 5 mg daily, increase to 10 mg after 7 days; maximum 20 mg/day

• Social anxiety disorder: Start at 10 mg daily, adjust after 2–4 weeks; usual dose 5–20 mg/day

Elderly patients:

• Usually 10 mg once daily; lower doses may be sufficient

• Administer cautiously; monitor for sensitivity

Important:

• Dose adjustments should be gradual; abrupt discontinuation can cause withdrawal symptoms such as dizziness, irritability, and sensory disturbances.

Drug Interactions

• Avoid co-administration with MAOIs (must allow 2 weeks after stopping MAOI before starting Andepram, and vice versa)

• Contraindicated with pimozide

• Caution with drugs affecting serotonin levels to prevent serotonin syndrome
