Indications

Epicure 50 ml is indicated for:

• Partial-onset seizures in adults (16 years and older)

• Partial-onset seizures in pediatric patients (4–16 years) with dosing based on body weight

It is not recommended for seizure types other than partial-onset without specialist guidance.

Pharmacology

The exact mechanism of action of Epicure 50 ml is not fully understood, but it demonstrates a high and selective affinity for synaptic vesicle protein 2A (SV2A) in the brain. SV2A plays a role in epileptogenesis by modulating synaptic GABA release, which may stabilize neuronal excitability. Additionally, Epicure 50 ml is believed to inhibit sodium channels, further contributing to its anticonvulsant effects.

Dosage & Administration

• Adults (≥16 years): Start with 50 mg twice daily (100 mg/day). Depending on response and tolerability, the dose may be increased to 100 mg twice daily (200 mg/day) or reduced to 25 mg twice daily (50 mg/day).

• Pediatric patients (4–16 years): Dosing is weight-based and administered orally twice daily.

• Intravenous administration (adults only): Administer over 2–15 minutes when oral dosing is not possible. Inspect for particulate matter or discoloration before use.

Administration note: Injection is single-use only. Oral tablets should be taken with or without food, consistently at the same time each day.

Drug Interactions

• Rifampin: May decrease Epicure levels; consider dose increase.

• Carbamazepine: May increase metabolite exposure; monitor and adjust carbamazepine dose if needed.

• Phenytoin: Plasma levels may rise; monitor phenytoin levels.

• Levetiracetam: No added benefit observed with co-administration.

• Avoid concomitant use with other SV2A-binding drugs without physician guidance.

Contraindications

• Hypersensitivity to Epicure or any inactive ingredients

Common adverse reactions (≥5% of patients) include:

• Somnolence or sedation

Precautions & Warnings

• Suicidal behavior/ideation: Monitor patients.

• Neurological reactions: Advise caution with driving or operating machinery.

• Psychiatric reactions: Monitor for irritability, aggression, depression, or psychotic symptoms.

• Hypersensitivity: Discontinue immediately if bronchospasm or angioedema occurs.

• Withdrawal: Gradual taper recommended; avoid abrupt discontinuation.

Use in Special Populations

• Hepatic impairment: Dose adjustment recommended.

• Renal impairment: No adjustment necessary.

• Pediatric patients: Safety for injection not established; oral dosing is weight-based.

• Elderly: Monitor for sedation, dizziness, and increased fall risk.

Storage Conditions
