I-Guard 0.5% is a broad-spectrum antimicrobial preparation used in ophthalmic, otic, and systemic bacterial infections. It belongs to the macrolide group of antibiotics and is effective against a wide range of gram-positive, gram-negative, and anaerobic organisms. Due to its extensive antimicrobial coverage, I-Guard 0.5% plays a significant role in the management of severe and life-threatening infections as well as localized eye and ear infections.

I-Guard 0.5% is indicated for the treatment of a wide variety of infections, including:
Ocular infections
Otitis externa
Bacterial meningitis
Anaerobic bacterial infections
Anthrax
Brain abscess
Ehrlichiosis
Gas gangrene
Granuloma inguinale
Infections caused by Haemophilus influenzae
Listeriosis
Plague
Psittacosis
Q fever
Severe gastroenteritis
Severe melioidosis
Severe systemic infections caused by Campylobacter fetus
Tularemia
Whipple’s disease
I-Guard 0.5% exerts its antibacterial effect by inhibiting bacterial protein synthesis. It binds to the 50S subunit of the bacterial ribosome, thereby preventing peptide bond formation through inhibition of peptidyl transferase. This action suppresses bacterial growth and replication. Depending on the organism and concentration achieved at the site of infection, I-Guard 0.5% may exhibit both bacteriostatic and bactericidal activity. It is particularly active against H. influenzae, Neisseria meningitidis, and Streptococcus pneumoniae.
Ophthalmic Use:
Adults, children, and infants: Instill 1–2 drops into the affected eye(s) 4–6 times daily.
The frequency may be increased if necessary.
Treatment usually continues for approximately 7 days and should not exceed 3 weeks without medical re-evaluation.
Otic Use:
Instill 2–3 drops into the ear canal three to four times daily.
For otitis externa: 2–3 drops into the ear twice to three times daily.
Oral Use (Systemic Infections):
Adults: 50 mg/kg/day in four divided doses, increased to 100 mg/kg/day in severe infections or meningitis.
Children: 50 mg/kg/day in four divided doses, increased to 100 mg/kg/day in severe infections.
Neonates: Dose adjustments based on age and weight are required.
Reduced efficacy with phenobarbitone and rifampicin
Decreased absorption of iron and vitamin B12 in anemic patients
May impair the effectiveness of oral contraceptives
Hypersensitivity to the drug
Pregnancy and lactation
Porphyria
Pre-existing bone marrow depression or blood dyscrasias
Gastrointestinal disturbances
Neurological effects such as headache, confusion, or encephalopathy
Hemolysis in patients with G6PD deficiency
Hypersensitivity reactions with ophthalmic use
Ototoxicity with ear drops
Pregnancy Category C. I-Guard 0.5% should be used only when the potential benefits outweigh the possible risks to the fetus.
Capsules/suspensions: Store below 30°C
Ophthalmic and otic preparations: Store at 2–8°C
Do not freeze and protect from light.
