Topical 5% Gel:
Intaflam 400 mg 5% gel is indicated for local relief of pain and inflammation associated with:
Backache
Rheumatic and muscular pain
Sprains and strains
Sports injuries
Non-serious arthritic conditions
Neuralgia (nerve pain)
Intaflam 400 mg is a non-selective cyclooxygenase (COX) inhibitor that blocks the synthesis of prostaglandins via the arachidonic acid pathway. Its primary anti-inflammatory, analgesic, and antipyretic effects are mainly attributed to inhibition of COX-2, reducing inflammation, pain, fever, and swelling. Inhibition of COX-1 is associated with certain adverse effects, particularly gastrointestinal irritation.
Intaflam 400 mg is administered as a racemic mixture, with in vivo conversion of the R-enantiomer to the more pharmacologically active S-enantiomer.
Oral Administration:
Adults:
Arthritic conditions: 400 mg, 3–4 times daily, preferably after food
Maximum daily dose: 2400 mg
Mild to moderate pain: 400 mg every 6 hours as required
Dysmenorrhoea: 400 mg every 4 hours as needed
Children:
Usual dose: 20–30 mg/kg body weight daily in divided doses
Maximum daily dose should not exceed 2400 mg
Not recommended for children under 1 year or weighing less than 5 kg
If gastrointestinal discomfort occurs, the medicine should be taken with food or milk.
Topical Administration:
Apply 4–10 cm of Intaflam 400 mg 5% gel to the affected area
Massage gently until absorbed
Use up to 4 times daily, with at least 4 hours between applications
For external use only on healthy, unbroken skin
Not recommended for children under 12 years
Increased risk of gastrointestinal bleeding with warfarin, corticosteroids, SSRIs, and aspirin
Reduced effectiveness of diuretics, ACE inhibitors, and angiotensin II receptor blockers
Increased toxicity of lithium and methotrexate
Increased risk of nephrotoxicity with ciclosporin and tacrolimus
Known hypersensitivity to Intaflam 400 mg or other NSAIDs
Active or severe peptic ulcer disease
Commonly reported side effects include:
Upset stomach
Nausea
Vomiting
Heartburn
Intaflam 400 mg is not recommended during pregnancy or while breastfeeding, unless clearly necessary and prescribed by a physician.
Use the lowest effective dose in patients with a history of gastrointestinal ulcer or bleeding
Long-term use requires periodic ocular monitoring
Use with caution in patients with asthma, systemic lupus erythematosus, or hypersensitivity to other NSAIDs
There is no specific antidote for Intaflam 400 mg. Management includes gastric lavage and correction of electrolyte imbalance if required.
Store in a cool, dry place, protected from light.
Keep out of reach of children.
