Linezolid is a bacteriostatic antibiotic that inhibits the formation of the bacterial initiation complex for protein synthesis. By binding to the 23S ribosomal RNA of the 50S subunit, it prevents the assembly of functional ribosomes, thereby blocking bacterial growth.
Linzolid exhibits excellent oral bioavailability (approximately 100%), allowing oral tablets to achieve plasma concentrations comparable to intravenous administration. It is widely distributed in tissues, including the lungs, skin, and soft tissues, making it effective in treating respiratory and skin infections.
Linezolid is metabolized primarily through non-enzymatic oxidation in the liver, producing inactive metabolites. Its half-life is approximately 4.5 to 5.5 hours, allowing twice-daily dosing. The drug is excreted through the kidneys, with 30–40% eliminated unchanged in the urine.
Linzolid 400 mg tablets are indicated for the treatment of:
Complicated and uncomplicated skin and soft tissue infections, including MRSA-associated infections
Nosocomial and community-acquired pneumonia caused by Gram-positive organisms
Infections due to vancomycin-resistant Enterococcus faecium (VRE)
Other serious Gram-positive bacterial infections where alternative treatments are limited
Adults: 400 mg orally every 12 hours for 10–14 days, depending on the severity and type of infection
Children: Dose should be calculated based on body weight and clinical condition (use under medical supervision)
Tablets can be taken with or without food
Duration of therapy should be based on clinical response and culture sensitivity
Known hypersensitivity to Linezolid or other oxazolidinones
Concurrent use of monoamine oxidase inhibitors (MAOIs) or within 2 weeks of MAOI therapy due to risk of serotonin syndrome
Severe uncontrolled hypertension in patients receiving serotonergic or adrenergic agents
Common side effects of Linzolid 400 mg include:
Gastrointestinal: nausea, vomiting, diarrhea, and taste disturbance
Hematologic: reversible thrombocytopenia, anemia, or neutropenia with prolonged use
Nervous system: headache, dizziness, insomnia, or peripheral neuropathy with long-term therapy
Rare but serious: lactic acidosis, serotonin syndrome, or optic neuropathy
Monitor complete blood count for therapy exceeding 2 weeks
Use with caution in patients with renal or hepatic impairment
Avoid tyramine-rich foods and concurrent serotonergic drugs to reduce the risk of hypertensive crises or serotonin syndrome
Use only if clearly indicated; limited data on safety in pregnancy
Linezolid is excreted in breast milk; breastfeeding should be discontinued or avoided during therapy
