Maxima MUPS 40 mg is a potent proton pump inhibitor designed to manage conditions caused by excessive gastric acid secretion. It is primarily used for the treatment and maintenance of gastroesophageal reflux disease (GERD), healing of erosive esophagitis, reduction of NSAID-associated gastric ulcers, H. pylori eradication in combination therapy, and management of Zollinger-Ellison syndrome.

Proton Pump Inhibitor (PPI)
Maxima MUPS 40 mg is indicated for:
Treatment of Gastroesophageal Reflux Disease (GERD)
Healing of erosive esophagitis
Maintenance of healing of erosive esophagitis
Symptomatic GERD
Reduction of NSAID-associated gastric ulcers
H. pylori eradication (as part of triple therapy)
Zollinger-Ellison syndrome
Maxima MUPS 40 mg works by selectively inhibiting the H+/K+-ATPase enzyme in the stomach lining, preventing the final step in gastric acid production. This leads to a significant reduction in both basal and stimulated acid secretion. Due to its irreversible binding to the enzyme, the antisecretory effect persists for more than 24 hours, allowing effective acid control.
Oral (Tablet/Capsule):
Healing erosive esophagitis: 20–40 mg once daily for 4–8 weeks. Patients who do not heal may continue for an additional 4–8 weeks.
Maintenance therapy: 20 mg once daily; clinical studies support up to six months.
Symptomatic GERD: 20 mg once daily for 4 weeks; may extend another 4 weeks if needed.
H. pylori eradication (Triple therapy): Maxima MUPS 40 mg 20 mg twice daily for 10 days, combined with Amoxicillin 1000 mg twice daily and Clarithromycin 500 mg twice daily.
Intravenous Injection:
Dose: 20–40 mg once daily for GERD with erosive esophagitis.
Reconstitution: Add 5 ml of 0.9% Sodium Chloride to the vial and dilute to a final volume of 50 ml.
Administration: 20 mg dose — withdraw 25 ml and infuse over 10–30 minutes; 10 mg dose — withdraw 12.5 ml and infuse over 10–30 minutes.
Exclude malignancy in patients with suspected gastric ulcers before starting treatment.
Monitor combination therapy with antibiotics according to their guidelines.
Avoid use in pregnancy and lactation unless benefits outweigh risks (US FDA Pregnancy Category C).
Hypersensitivity to any component is a contraindication.

Maxima MUPS 40 mg may affect hepatic clearance due to selective inhibition of the cytochrome P450 system, though clinically significant interactions are rare.
May influence absorption of vitamin B12 and potentially affect the efficacy of oral contraceptives.
Store in a dry, cool place away from light and heat.
Keep out of reach of children.
Prepare injection by adding 5 ml of 0.9% Sodium Chloride to the vial; resulting solution is clear and colorless to slightly yellow.
