Primace 5 mg Capsule is indicated for:
Hypertension: To lower high blood pressure, either alone or in combination with other antihypertensive agents
Heart failure: As part of the long-term management of patients with symptomatic heart failure
Post-myocardial infarction: To improve survival and reduce cardiovascular complications
Prevention of cardiovascular events: In patients with established cardiovascular risk factors, such as diabetes or a history of myocardial infarction
Diabetic nephropathy and chronic kidney disease: To slow the progression of renal impairment in patients with type 2 diabetes or proteinuric kidney disease
Ramipril is a prodrug converted in the liver to its active metabolite, ramiprilat, which inhibits the angiotensin-converting enzyme (ACE). By preventing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor, Ramipril promotes vasodilation, reduces aldosterone secretion, and decreases sodium and water retention. This results in lowered blood pressure, reduced strain on the heart, and improved perfusion to vital organs. Its renoprotective effects make it particularly beneficial in patients with diabetic kidney disease or chronic kidney disease.
Adults with hypertension: Usually started at 2.5–5 mg once daily; titrated according to response
Heart failure: Initial dose typically 1.25–2.5 mg once daily; gradually increased based on tolerance
Post-myocardial infarction: Initiated under careful monitoring of blood pressure and renal function
Elderly or patients with renal impairment: Start with lower doses (1.25–2.5 mg once daily) to minimize the risk of hypotension
Primace should be taken orally, with or without food, preferably at the same time each day. Capsules should be swallowed whole with water.
Diuretics: May enhance hypotensive effects
Potassium-sparing diuretics or potassium supplements: May increase risk of hyperkalemia
NSAIDs: Can reduce the antihypertensive effect and increase the risk of kidney dysfunction
Lithium: ACE inhibitors may elevate lithium levels, increasing toxicity risk
Known hypersensitivity to Ramipril or other ACE inhibitors
History of angioedema related to previous ACE inhibitor therapy
Pregnancy, especially during the second and third trimesters
Severe renal impairment or bilateral renal artery stenosis
Common adverse effects include dizziness, hypotension, headache, persistent dry cough, fatigue, and gastrointestinal discomfort. Rare but serious side effects include angioedema, hyperkalemia, and renal impairment.
