Relentus 2 mg is indicated for:
Relief of Painful Muscle Spasms:
Associated with static and functional disorders of the spine, including cervical and lumbar syndromes, and post-surgical recovery such as after herniated disc surgery or hip osteoarthritis.
Spasticity Due to Neurological Disorders:
Conditions like multiple sclerosis, chronic myelopathy, degenerative spinal cord diseases, cerebral palsy, and cerebrovascular accidents.
Tizanidine is a short-acting α2-adrenergic agonist that acts primarily at the spinal cord. It increases presynaptic inhibition of motor neurons, reducing excitatory neurotransmitter release and limiting polysynaptic reflexes. Unlike direct-acting muscle relaxants, tizanidine does not act on skeletal muscle fibers or neuromuscular junctions. Its antispastic effects are most pronounced on polysynaptic pathways, resulting in reduced muscle tone, decreased spasms, and improved voluntary movement. Additionally, tizanidine has moderate central analgesic properties, providing pain relief alongside muscle relaxation.
Stimulates presynaptic α2 receptors in the spinal cord to inhibit excitatory amino acid release
Reduces polysynaptic signal transmission responsible for excessive muscle tone
Decreases spasticity, muscle resistance, and clonus while moderately relieving pain
Initial Dose: 2 mg once daily
Titration: Increase in 2 mg increments every 3–4 days as needed, up to 24 mg daily in 3–4 divided doses
Maximum Dose: 36 mg daily
For Painful Muscle Spasms: 2–4 mg three times daily; additional nighttime doses may be given for severe cases
For Neurological Spasticity: Start with ≤6 mg/day divided into 3 doses; optimum response usually achieved at 12–24 mg/day
Special Populations:
Elderly: Start at lowest dose, titrate carefully
Renal Impairment: Start at 2 mg once daily, increase cautiously
Hepatic Impairment: Contraindicated in severe cases; use caution in moderate impairment
Discontinuation should be gradual to avoid rebound hypertension and tachycardia.
CYP1A2 inhibitors (fluvoxamine, ciprofloxacin): Contraindicated due to risk of severe hypotension and prolonged tizanidine levels
Other CYP1A2 inhibitors: Use with caution (e.g., amiodarone, cimetidine)
Drugs prolonging QT interval: Caution advised
Antihypertensives: May enhance hypotensive effects
Alcohol, sedatives, hypnotics: May potentiate CNS depressant effects
Hypersensitivity to tizanidine
Severe hepatic impairment
Concomitant use with strong CYP1A2 inhibitors
Very common: somnolence, dizziness, fatigue, muscle weakness, gastrointestinal disorders
Common: hypotension, bradycardia, nausea, dry mouth, increased liver enzymes
Rare: hallucinations, sleep disorders, hepatitis, hypersensitivity reactions
Limited human data; use only if benefit outweighs risk
Animal studies: No teratogenic effects but possible fetal growth and developmental effects at high doses
Not recommended during breastfeeding
Monitor liver function monthly for high doses
Avoid abrupt discontinuation to prevent rebound hypertension
Patients experiencing sedation or hypotension should avoid driving or operating machinery
Use caution in elderly and renally impaired patients
Symptoms: hypotension, bradycardia, somnolence, respiratory depression, QT prolongation, coma
Management: activated charcoal, supportive care, and forced diuresis
Store in a dry place below 30°C, away from light and moisture.
