Tibolone itself is pharmacologically inactive. After oral administration, it is rapidly metabolized into three active metabolites. Two of these metabolites have estrogenic activity, which helps relieve hot flushes and prevent bone loss, while the third metabolite has progestogenic and androgenic activity, contributing to protection of the endometrium and improvement in libido and mood.

Unlike conventional estrogen–progestogen combinations, tibolone exerts tissue-selective effects, acting as an estrogen in bone, brain, and vagina, while having minimal stimulatory effects on the endometrium and breast tissue. This selective activity reduces the incidence of withdrawal bleeding and breast tenderness compared with traditional HRT.

Indications

Tibilon Tablet 2.5 mg is indicated for:

• Relief of menopausal symptoms such as hot flushes, night sweats, mood changes, and vaginal dryness

• Prevention of postmenopausal osteoporosis in women at increased risk of fractures who cannot tolerate or are contraindicated for other osteoporosis therapies

Dosage and Administration

The recommended dose is one tablet (2.5 mg) once daily, taken orally at the same time each day.

• Treatment should begin at least 12 months after the last natural menstrual period.

• Tablets may be taken with or without food.

• If a dose is missed, it should be taken as soon as remembered unless more than 12 hours have passed; in that case, the missed dose should be skipped.

Drug Interactions

• Enzyme inducers such as rifampicin, carbamazepine, and phenytoin may reduce the effectiveness of tibolone.

• Caution is advised when used with anticoagulants, as tibolone may enhance their effects.

• Concomitant use with other hormonal therapies is generally not recommended.

Contraindications

Tibilon Tablet 2.5 mg is contraindicated in patients with:

• Known or suspected breast cancer or estrogen-dependent malignancies

• Undiagnosed vaginal bleeding

• Active or recent thromboembolic disorders

• Severe liver disease

• Known hypersensitivity to tibolone

Side Effects

Common side effects may include:

• Abdominal pain

• Weight gain

• Breast discomfort

• Vaginal spotting or discharge

• Headache

Most adverse effects are mild and tend to decrease with continued therapy.

Pregnancy and Lactation

• Pregnancy: Tibolone is contraindicated during pregnancy.

• Lactation: Not indicated for use in breastfeeding women.

Precautions and Warnings

• Use the lowest effective dose for the shortest duration consistent with treatment goals

• Periodic medical evaluation, including breast and pelvic examinations, is recommended

• Caution is advised in women with cardiovascular disease risk factors
