Vilanterol Trifenatate is a selective long-acting beta₂-adrenergic receptor agonist that relaxes bronchial smooth muscle by stimulating adenyl cyclase and increasing intracellular cyclic AMP. This results in rapid onset and prolonged bronchodilation, improving airflow and reducing dyspnea.

Umeclidinium Bromide is a long-acting muscarinic antagonist that blocks the action of acetylcholine on M₃ receptors in the airways. By preventing bronchoconstriction and reducing mucus secretion, it provides sustained airway relaxation and improved respiratory capacity.

Fluticasone Furoate is a potent inhaled corticosteroid with strong anti-inflammatory activity. At the 200 mcg strength, it offers enhanced suppression of airway inflammation by inhibiting inflammatory cell migration, cytokine release, and mucosal edema. This reduces airway hyperresponsiveness and significantly lowers the frequency and severity of COPD exacerbations.

The combined pharmacological actions deliver superior symptom control and long-term disease management compared to mono or dual bronchodilator therapy.

Indications

Trilexa Inhalation Capsule is indicated for:

• Maintenance treatment of chronic obstructive pulmonary disease (COPD)

• Patients with moderate to severe COPD requiring triple therapy

• Reduction of COPD exacerbations in patients with frequent flare-ups

This medication is not intended for the relief of acute bronchospasm.

Dosage & Administration

• The recommended dose is one inhalation capsule once daily, administered using an appropriate dry powder inhaler device, or as directed by a physician.

• Capsules are for inhalation only and must not be swallowed.

• Regular daily use is essential for optimal therapeutic benefit.

• Patients should rinse their mouth after inhalation to reduce the risk of oral candidiasis.

Dosage should be individualized based on disease severity and clinical response.

Contraindications

• Known hypersensitivity to Vilanterol, Fluticasone Furoate, Umeclidinium, or any excipients

• Use as initial therapy for acute asthma attacks or severe bronchospasm

Side Effects

Trilexa Inhalation Capsule is generally well tolerated. Possible side effects include:

• Headache and dizziness

• Dry mouth or throat irritation

• Cough or hoarseness

• Palpitations or tremor

• Oral candidiasis with prolonged use

Most adverse effects are mild and transient.

Precautions & Warnings

• Use with caution in patients with cardiovascular disease, diabetes mellitus, glaucoma, or urinary retention

• Inhaled corticosteroids may increase the risk of pneumonia in COPD patients

• Do not exceed the recommended dose

• Not suitable as a rescue medication for sudden breathing difficulties
