Vanmycin Injection 500 mg/vial, containing Vancomycin Hydrochloride, is a potent antibiotic indicated for the treatment of serious infections caused by Gram-positive bacteria. It is primarily used in cases where other antibiotics, such as beta-lactams, are ineffective or contraindicated. The injection is commonly prescribed for:

• Severe infections caused by methicillin-resistant Staphylococcus aureus (MRSA)

• Infections caused by Staphylococcus epidermidis, Streptococcus species, and Enterococcus species

• Endocarditis, including bacterial endocarditis in patients allergic to penicillin

• Pneumonia, including hospital-acquired and ventilator-associated infections

• Skin, soft tissue, bone, and joint infections

• Septicemia and bloodstream infections

• Infections caused by Clostridium difficile (for certain cases of intestinal infection, though oral formulations are preferred for gastrointestinal infections)

Vancomycin is also used prophylactically in patients undergoing certain types of surgery, particularly in those at high risk for MRSA infections.

Pharmacology

Vancomycin is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis. By binding to the D-alanyl-D-alanine terminus of cell wall precursor units, it prevents proper cross-linking and weakens the bacterial cell wall, leading to cell lysis and death. Vancomycin exhibits bactericidal activity specifically against Gram-positive organisms, including MRSA, penicillin-resistant Streptococcus pneumoniae, and Clostridium species.

The drug is poorly absorbed orally, which is why parenteral administration is necessary for systemic infections. Intravenous administration achieves therapeutic plasma levels rapidly, making it effective for severe and life-threatening infections. Vancomycin is primarily eliminated by renal excretion; thus, dose adjustment is required in patients with impaired kidney function.

Dosage & Administration

Vanmycin Injection should only be administered under medical supervision. The dosage depends on the type and severity of the infection, the patient’s age, body weight, and renal function.

• Adults: Typical dosage ranges from 15–20 mg/kg body weight per day, divided into 2–4 doses, infused intravenously over at least 60 minutes to minimize infusion-related reactions.

• Children: Dosage is usually 40 mg/kg/day, divided every 6–8 hours, with adjustments based on renal function and infection severity.

• Renal Impairment: Dosage should be modified according to creatinine clearance and serum vancomycin levels to avoid accumulation and toxicity.

Close monitoring of serum vancomycin concentrations, kidney function, and hearing is recommended during therapy.

Contraindications

Vanmycin Injection is contraindicated in patients with known hypersensitivity to vancomycin or any of its excipients.

Side Effects

Common adverse effects may include:

• Red man syndrome (flushing, rash, and hypotension during infusion)

• Fever, chills, or phlebitis at the injection site

• Nephrotoxicity, particularly with prolonged therapy or high doses

• Ototoxicity (hearing loss or tinnitus), especially in patients with pre-existing auditory impairment

• Rarely, neutropenia or thrombocytopenia

Infusion reactions can usually be managed by slowing the infusion rate and pre-medicating with antihistamines if necessary.
