Zolenic Injection 5 mg/100 ml is a potent bisphosphonate preparation containing Zoledronic Acid. It is primarily used to treat osteoporosis and other bone-related disorders by inhibiting osteoclast-mediated bone resorption. Administered intravenously, Zolenic works directly on mineralized bone and targets farnesyl pyrophosphate synthase in osteoclasts, helping to strengthen bones and reduce fracture risk.

Indications
Zolenic Injection 5 mg/100 ml is indicated for:
Treatment of postmenopausal osteoporosis to reduce the risk of hip, vertebral, and non-vertebral fractures.
Prevention of clinical fractures following a hip fracture.
Treatment and prevention of glucocorticoid-induced osteoporosis.
Treatment of osteoporosis in men.
Management of Paget’s disease of bone.
Note: Treatment should be limited to three annual doses.

Pharmacology
Zoledronic Acid, the active ingredient, is a nitrogen-containing bisphosphonate that binds strongly to bone mineral and inhibits bone resorption. It is distributed rapidly to bone after intravenous administration, is not metabolized, and is excreted unchanged by the kidneys. The primary molecular target is farnesyl pyrophosphate synthase in osteoclasts, though other mechanisms may contribute.
Dosage and Administration
Postmenopausal osteoporosis, osteoporosis in men, glucocorticoid-induced osteoporosis: Single IV infusion of 5 mg once yearly. Adequate supplemental Calcium and Vitamin-D intake is recommended.
Prevention of fractures after hip fracture: Single IV infusion of 5 mg once yearly, ideally two or more weeks after hip fracture repair. A loading dose of 50,000–125,000 IU Vitamin-D is recommended prior to infusion.
Paget’s disease of bone: Single IV infusion of 5 mg. Re-treatment may be considered based on clinical or biochemical indicators 12 months after the initial dose. Ensure adequate Calcium (≥500 mg twice daily for 10 days) and Vitamin-D supplementation.
Administration Guidelines: Infuse over at least 15 minutes via a separate IV line at a constant rate. Do not mix with other medications or divalent cation-containing solutions.

Contraindications
Hypersensitivity to Zoledronic Acid, excipients, or other bisphosphonates
Hypocalcaemia
Renal impairment (creatinine clearance <35 mL/min)
Current/recent uveitis or history of bisphosphonate-associated uveitis
Pregnancy and lactation
Precautions and Warnings
Ensure proper hydration before infusion, especially in elderly patients and those on diuretics.
Correct pre-existing hypocalcaemia and other mineral metabolism disorders.
Monitor renal function in at-risk patients.
Not recommended for children or adolescents under 18 years of age.
Side Effects
Common side effects occurring within the first 3 days include:
Fever, myalgia, flu-like symptoms, arthralgia, headache
Severe bone, joint, or muscle pain (rare)
Note: Mild post-dose symptoms may be mitigated with Paracetamol or Ibuprofen.
Drug Interactions
Caution when used with drugs affecting renal function, such as aminoglycosides or dehydrating diuretics. No formal drug-drug interaction studies have been conducted.
Overdose
Monitor patients closely. Clinically significant hypocalcaemia may be treated with supplemental Calcium orally or intravenously.
Storage Conditions
Store below 30°C, protected from moisture and light. Keep out of reach of children. Once opened, use immediately or store at 2–8°C for up to 24 hours.
Use in Special Populations
Renal impairment: Not recommended if creatinine clearance <35 mL/min
Hepatic impairment: No dose adjustment needed
Elderly: Monitor renal function; no dose adjustment required
