Acos 500 mg Tablet is a macrolide antibiotic belonging to the azalide subgroup. It is widely prescribed for the treatment of various bacterial infections caused by susceptible organisms. Acos works by inhibiting bacterial protein synthesis, thereby preventing the growth and multiplication of infection-causing bacteria.
This antibiotic is effective against a broad range of gram-positive and gram-negative bacteria, as well as certain atypical pathogens. It is commonly used for respiratory tract infections, skin infections, and specific sexually transmitted diseases.

Macrolide Antibiotic (Azalide group)
Acos 500 mg is indicated for the treatment of infections caused by susceptible microorganisms, including:
Upper respiratory tract infections such as sinusitis and pharyngitis/tonsillitis
Lower respiratory tract infections including bronchitis and pneumonia
Otitis media
Skin and soft tissue infections
Non-gonococcal urethritis and cervicitis caused by Chlamydia trachomatis
Certain sexually transmitted infections in men and women
Typhoid fever (as prescribed)
Acos 500 mg acts by binding to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting microbial protein synthesis. This action prevents bacterial growth and replication.
It demonstrates activity against a wide range of organisms including:
Gram-positive bacteria such as Streptococcus and Staphylococcus aureus
Gram-negative bacteria including Haemophilus influenzae, Moraxella catarrhalis, and Neisseria gonorrhoeae
Atypical pathogens such as Mycoplasma pneumoniae and Legionella pneumophila
Other organisms including Campylobacter species and Treponema pallidum
500 mg once daily for 3 days
OR
500 mg on Day 1, followed by 250 mg once daily on Days 2–5
For sexually transmitted infections caused by Chlamydia trachomatis:
1 g as a single dose
OR
500 mg on Day 1, followed by 250 mg once daily for the next 2 days
10 mg/kg body weight once daily for 3 days
Specific dosing according to body weight:
15–25 kg: 200 mg once daily for 3 days
26–35 kg: 300 mg once daily for 3 days
36–45 kg: 400 mg once daily for 3 days
For typhoid fever:
500 mg once daily for 7–10 days (as directed by physician)
Shake the bottle thoroughly to loosen the powder.
Add boiled and cooled water up to the marked line.
Shake well until fully mixed.
Take Acos 500 mg at least 1 hour before or 2 hours after meals.

Food and antacids may reduce absorption.
Avoid concomitant use with ergot alkaloids due to risk of ergotism.
Use caution when co-administered with cyclosporine or digoxin as plasma levels may increase.
No significant interaction reported with warfarin, theophylline, carbamazepine, methylprednisolone, or cimetidine.
Hypersensitivity to Acos or other macrolide antibiotics
Concurrent use with ergot derivatives
Patients with significant liver disorders
Side effects are generally mild and reversible. These may include:
Nausea
Vomiting
Abdominal pain or cramps
Diarrhea
Flatulence
Headache
Skin rash
Pregnancy Category B
Use during pregnancy only if clearly needed and no suitable alternative is available.
Caution is advised during breastfeeding as excretion in breast milk is unknown.
Monitor for signs of superinfection (e.g., fungal infection) during prolonged use.
No dose adjustment is required in patients with renal impairment.
Use cautiously in patients with liver dysfunction.
Symptoms of macrolide overdose may include hearing loss, severe nausea, vomiting, and diarrhea. Gastric lavage and supportive management are recommended.
Store in a cool, dry place away from heat and light.
Keep out of reach of children.
