Carlina-M Tablet 2.5 mg + 500 mg is a combination oral antidiabetic medication containing Linagliptin 2.5 mg and Metformin Hydrochloride 500 mg. This combination is designed for adults with type 2 diabetes mellitus who require better blood glucose control. By combining two complementary mechanisms of action, Carlina-M helps reduce both fasting and postprandial blood sugar levels while supporting overall glycemic management.
Linagliptin is a DPP-4 (dipeptidyl peptidase-4) inhibitor, which enhances the body’s natural incretin hormones. These hormones increase insulin secretion in response to elevated blood glucose and suppress glucagon release from the pancreas, helping lower blood sugar levels.
Metformin Hydrochloride is a biguanide class antidiabetic that reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity in peripheral tissues. The combination of these two agents provides a synergistic effect on blood glucose regulation, making it easier to achieve target HbA1c levels in patients inadequately controlled by diet, exercise, or monotherapy.
Carlina-M is especially suitable for patients who need dual therapy to manage hyperglycemia while minimizing the risk of hypoglycemia and weight gain.

Oral Antidiabetic Agent
DPP-4 Inhibitor + Biguanide Combination
Carlina-M Tablet is indicated for:
Adults with type 2 diabetes mellitus inadequately controlled with diet and exercise
Patients requiring dual therapy to achieve glycemic targets
Can be used as add-on therapy for patients already on Metformin or Linagliptin monotherapy
Helps in reducing fasting and postprandial glucose levels
It is not indicated for type 1 diabetes or diabetic ketoacidosis.
Linagliptin:
Inhibits DPP-4 enzyme, preventing the breakdown of incretin hormones
Incretins increase insulin secretion in response to glucose
Suppresses glucagon secretion, reducing hepatic glucose output
Metformin Hydrochloride:
Decreases hepatic gluconeogenesis
Reduces intestinal glucose absorption
Improves insulin sensitivity in muscle and fat tissues
The combination provides complementary and synergistic effects, resulting in improved glycemic control with low risk of hypoglycemia.
Usually taken twice daily with meals to reduce gastrointestinal side effects
Swallow the tablet whole with water; do not crush or chew
Dosage may be adjusted based on blood glucose levels, renal function, and physician’s advice
Regular monitoring of blood glucose, HbA1c, and renal function is recommended
Carlina-M is generally well tolerated. Common side effects may include:
Gastrointestinal discomfort (nausea, vomiting, diarrhea, abdominal pain)
Headache
Mild dizziness
Rare but serious side effects may include lactic acidosis (primarily from Metformin), hypoglycemia (especially when used with other antidiabetic agents), or allergic reactions. Patients should seek medical attention if severe symptoms occur.
Avoid in patients with severe renal impairment, hepatic dysfunction, or metabolic acidosis
Monitor renal function before and during therapy
Discontinue before radiologic studies involving iodinated contrast media
Not recommended during pregnancy or lactation unless prescribed by a physician
Adherence to diet and lifestyle modifications is essential
Store in a cool, dry place below 30°C
Keep the container tightly closed
