Feelnor MR 35 mg is indicated in adults as add-on therapy for the symptomatic treatment of stable angina pectoris, particularly in patients who remain symptomatic despite standard anti-anginal treatment or cannot tolerate first-line therapies.
Trimetazidine is a 3-keto acyl-CoA thiolase (KAT) inhibitor, making it a unique metabolic anti-ischemic agent. It works by partially inhibiting fatty acid oxidation in cardiac cells and shifting energy production toward glucose oxidation. Since glucose metabolism requires less oxygen to generate the same amount of energy, this metabolic shift improves myocardial efficiency under ischemic conditions. Additionally, enhanced glucose oxidation reduces lactic acid accumulation, helping to prevent angina-related chest pain and improve exercise tolerance.
Feelnor MR 35 mg inhibits the fatty acid metabolic pathway by blocking the 3-keto acyl-CoA thiolase enzyme. This promotes glucose oxidation, which is a more oxygen-efficient energy pathway. As a result, the heart produces more energy from the same oxygen supply, improving myocardial function and reducing ischemia-related symptoms such as angina pectoris.
Recommended dose: Trimetazidine 35 mg twice daily during meals
Alternatively, 20 mg three times daily may be used
Treatment response should be evaluated after three months
Discontinue therapy if no clinical benefit is observed
Dosage adjustment may be required in patients with renal impairment.
No clinically significant drug interactions have been reported. Trimetazidine has been safely used alongside beta-blockers, calcium antagonists, nitrates, heparin, hypolipidemic agents, and digitalis preparations.
Feelnor MR 35 mg is contraindicated in patients with:
Hypersensitivity to Trimetazidine or any excipients
Parkinson’s disease or parkinsonian symptoms
Tremors or restless legs syndrome
Severe renal impairment
Trimetazidine is generally safe and well tolerated. Common side effects may include:
Dizziness and headache
Abdominal pain, diarrhoea, dyspepsia
Nausea and vomiting
Skin reactions such as rash, pruritus, urticaria
Asthenia
Most adverse effects are mild and reversible.
There is limited data on the use of Trimetazidine in pregnant women. Animal studies do not indicate reproductive toxicity, but as a precaution, its use during pregnancy is not recommended. It is unknown whether Trimetazidine is excreted in human milk; therefore, it should not be used during breastfeeding due to potential risk to the infant.
Feelnor MR 35 mg is not intended for acute angina attacks, unstable angina, or myocardial infarction. It should only be used as supportive therapy for stable angina under medical supervision.
