Non-sedating Antihistamines
Fenofex Syrup 50 ml is indicated for the treatment of:
Allergic rhinitis (seasonal and persistent) in adults and children
Sneezing
Rhinorrhea
Lacrimation
Itchy, red eyes
Itching of the nose, palate, and throat
Chronic idiopathic urticaria in adults and children
Reduction in wheals
Relief from pruritus
Fexofenadine is a selective peripheral H1-receptor antagonist. After oral administration, it is rapidly absorbed and reaches peak plasma concentrations within 2 to 3 hours. Unlike first-generation antihistamines, fexofenadine does not readily cross the blood-brain barrier, which explains its minimal sedative effect and low risk of cognitive impairment.
Allergic reactions occur when histamine is released from mast cells and basophils following allergen exposure. Histamine binds to H1 receptors, triggering symptoms such as itching, nasal discharge, and watery eyes. Fexofenadine acts as an inverse agonist at H1 receptors, stabilizing them in an inactive state and preventing the downstream inflammatory cascade. It has no significant anticholinergic, sedative, or central nervous system depressant effects.
Allergic Rhinitis and Chronic Idiopathic Urticaria
Adults and children ≥12 years:
60 mg twice daily or
120 mg once daily or
180 mg once daily
Children 2–11 years:
30 mg (5 ml) twice daily
Children 6 months to <2 years (urticaria):
15 mg (2.5 ml) twice daily
In renal impairment: reduced once-daily dosing is recommended
Dosage should be adjusted according to age, clinical response, and renal function.

Fenofex Syrup has minimal interaction potential as it does not undergo hepatic metabolism. Co-administration with erythromycin or ketoconazole may increase plasma levels without clinical significance. Antacids containing aluminum or magnesium hydroxide may reduce absorption; a 2-hour interval between dosing is advised.
Fenofex Syrup is contraindicated in patients with known hypersensitivity to fexofenadine or any of its ingredients.
Fenofex is generally well tolerated. Common side effects include:
Headache
Fatigue
Dizziness
Nausea
Rare post-marketing effects include hypersensitivity reactions, palpitations, insomnia, rash, urticaria, and pruritus.
Fenofex Syrup is classified as Pregnancy Category C. Its use during pregnancy and breastfeeding is not recommended unless clearly necessary and advised by a physician.
Caution is advised in elderly patients and those with renal impairment or cardiovascular disease. Fenofex is unlikely to impair the ability to drive or operate machinery, though individual sensitivity should be considered.
Store in a dry place away from heat and light. Keep out of the reach of children.
