Indications

Flutisal Inhalation Capsule is indicated for:

• Maintenance treatment of asthma in patients who require both an inhaled corticosteroid and a long-acting beta₂-agonist

• Long-term management of chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema

• Prevention of asthma symptoms such as wheezing, shortness of breath, chest tightness, and coughing

This product is not intended for the relief of acute bronchospasm or sudden asthma attacks.

Pharmacology

Flutisal combines two complementary agents with distinct mechanisms of action:

• Salmeterol is a long-acting selective beta₂-adrenergic receptor agonist. It works by relaxing the smooth muscles of the airways, leading to bronchodilation and improved airflow. Its prolonged duration of action helps maintain airway patency for up to 12 hours.

• Fluticasone Propionate is a synthetic inhaled corticosteroid with strong anti-inflammatory activity. It reduces airway inflammation by inhibiting multiple inflammatory cells and mediators involved in asthma and COPD, thereby decreasing mucosal edema, mucus production, and airway hyperresponsiveness.

Together, these agents provide both symptom control and anti-inflammatory protection.

Dosage & Administration

Flutisal Inhalation Capsule is for inhalation use only and should be administered using a suitable dry powder inhalation device.

• The usual dose is one capsule inhaled twice daily, or as directed by a physician

• Capsules should not be swallowed

• Regular use is essential to achieve optimal therapeutic benefit

Patients should rinse their mouth with water after inhalation to reduce the risk of oropharyngeal candidiasis.

Drug Interactions

Caution is advised when Flutisal is used with strong CYP3A4 inhibitors such as ketoconazole or ritonavir, as they may increase systemic exposure to Fluticasone Propionate. Concomitant use with other long-acting beta₂-agonists should be avoided. Beta-blockers may reduce the bronchodilatory effect of Salmeterol. Use with other corticosteroids may increase the risk of systemic side effects.

Contraindications

Flutisal Inhalation Capsule is contraindicated in patients with known hypersensitivity to Salmeterol, Fluticasone Propionate, or any component of the formulation. It should not be used as monotherapy for acute asthma attacks or status asthmaticus.

Side Effects

Commonly reported side effects include:

• Throat irritation or hoarseness

• Oral candidiasis

• Cough

Less common but potentially serious adverse effects include palpitations, tremor, hypokalemia, adrenal suppression, and paradoxical bronchospasm.

Pregnancy & Lactation

Flutisal should be used during pregnancy only if the potential benefit outweighs the possible risk to the fetus. Caution is advised when administered to breastfeeding mothers, as small amounts of corticosteroids may be excreted in breast milk.

Precautions & Warnings
