Gabapen 300 mg is indicated for the treatment of:

Pharmacology

Gabapen 300 mg is an anticonvulsant agent and a structural analog of gamma-aminobutyric acid (GABA). Although it resembles GABA chemically, it does not bind directly to GABA receptors nor does it influence GABA uptake or degradation.

All pharmacological actions of Gabapen 300 mg are attributed to the parent compound. Gabapentin binds selectively to the alpha-2-delta subunit of voltage-gated L-type calcium channels in the central nervous system. This binding reduces calcium influx into neurons, leading to inhibition of excitatory neurotransmitter release. Additionally, it inhibits branched-chain amino acid transferase, contributing to reduced neuronal excitability and improved control of neuropathic pain and seizures.

Dosage

Neuropathic Pain:

• Day 1: 300 mg once daily

• Day 2: 300 mg twice daily

• Day 3: 300 mg three times daily

The dose may be gradually increased based on clinical response up to a maximum of 1800 mg per day, divided into three doses.

Epilepsy:

• Patients over 12 years: Effective dose ranges from 900–1800 mg per day, given in three divided doses. The usual starting dose is 300 mg three times daily.

• Pediatric patients (3–12 years): Starting dose is 10–15 mg/kg/day in three divided doses, with gradual upward titration over approximately three days to reach the effective dose.

Dosage adjustments should be made under medical supervision.

Administration

Gabapen 300 mg can be taken orally with or without food. Capsules should be swallowed whole with water.

Drug Interaction

• Antacids may reduce the bioavailability of Gabapen 300 mg by up to 20%; it is recommended to separate administration times.

• Cimetidine may alter renal excretion of gabapentin.

• Gabapen 300 mg does not show significant interaction with other anti-epileptic drugs or oral contraceptives.

Contraindications

Gabapen 300 mg is contraindicated in patients with known hypersensitivity to gabapentin or any component of the formulation.

Commonly reported side effects include:

• Fatigue

• Dizziness

• Ataxia

• Somnolence

• Weight gain

• Peripheral edema

• Dry mouth

Most adverse effects are mild to moderate and usually resolve with continued therapy or dose adjustment.

Gabapen 300 mg is classified as Pregnancy Category C. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin may be excreted in breast milk; therefore, caution is advised during lactation, and use should be based on a risk–benefit assessment.

Precautions & Warnings

• Patients should take Gabapen 300 mg strictly as prescribed.

• Caution is advised when driving or operating machinery until the individual response to the medication is known.

• Dose reduction is required in patients with renal impairment.

• Abrupt discontinuation should be avoided to prevent withdrawal symptoms or seizure exacerbation.

Use in Special Populations

In patients with renal impairment, Gabapen 300 mg dosage must be reduced according to renal function.
