Klarix 70 ml is an oral antibiotic suspension containing Clarithromycin, a macrolide antibiotic with broad-spectrum activity. It is commonly prescribed for respiratory tract infections, skin and soft tissue infections, and as part of combination therapy for the eradication of Helicobacter pylori in patients with duodenal ulcers. Klarix 70 ml is suitable for both adult and pediatric patients and is well tolerated due to its improved acid stability compared to erythromycin.

Macrolides
Klarix 70 ml is indicated for the treatment of the following infections caused by susceptible organisms:
Streptococcal pharyngitis and tonsillitis
Acute and chronic sinusitis
Infective exacerbations of chronic bronchitis
Community-acquired pneumonia
Atypical pneumonia
Skin and soft tissue infections
Adjunctive therapy in the eradication of Helicobacter pylori in patients with duodenal ulcers
Clarithromycin acts by inhibiting bacterial protein synthesis. It binds to the 50S subunit of the bacterial ribosome, blocking translocation and thereby preventing bacterial growth. Klarix 70 ml is active against most Gram-positive organisms, Chlamydia, Mycoplasma, and some Gram-negative bacteria. It demonstrates equal or greater in vitro activity compared to erythromycin and is more acid-stable, resulting in improved gastrointestinal tolerability. Clarithromycin also shows enhanced activity against Haemophilus influenzae.
Adults:
Pharyngitis/Tonsillitis: 250 mg every 12 hours for 10 days
Acute maxillary sinusitis: 500 mg every 12 hours for 14 days
Chronic bronchitis: 250–500 mg every 12 hours for 7–14 days
Pneumonia: 250 mg every 12 hours for 7–14 days
Uncomplicated skin and skin structure infections: 250 mg every 12 hours for 7–14 days
Community-acquired upper and lower respiratory tract infections: 250–500 mg every 12 hours for 5–14 days
Children:
Body weight under 8 kg: 7.5 mg/kg twice daily
8–11 kg (1–2 years): 2.5 ml twice daily
12–19 kg (3–6 years): 5 ml twice daily
20–29 kg (7–9 years): 7.5 ml twice daily
30–40 kg (10–12 years): 10 ml twice daily
Dosage and duration should be determined by the physician based on infection severity and patient response.
Klarix 70 ml may be administered with or without food.
Theophylline: Concurrent use may increase serum theophylline concentrations
Terfenadine: Clarithromycin may alter the metabolism of terfenadine, increasing the risk of cardiac adverse effects
Hypersensitivity to clarithromycin, erythromycin, or other macrolide antibiotics
Patients receiving terfenadine with pre-existing cardiac abnormalities or electrolyte disturbances
Klarix 70 ml is generally well tolerated. Possible side effects include:
Nausea, vomiting, diarrhoea, and abdominal pain
Stomatitis and glossitis
Headache and taste disturbance
Allergic reactions ranging from mild skin reactions to anaphylaxis
Rare transient central nervous system effects such as dizziness, insomnia, anxiety, or hallucinations
Klarix 70 ml is not recommended during pregnancy due to limited clinical experience. Breastfeeding should be discontinued during treatment, as clarithromycin is excreted in breast milk.
Use with caution in patients with hepatic or renal impairment
Prolonged or repeated use may result in overgrowth of non-susceptible bacteria or fungi
Discontinue treatment and initiate appropriate therapy if superinfection occurs
Ingestion of large amounts may cause gastrointestinal symptoms. Rare cases of altered mental status and electrolyte disturbances have been reported. Management includes supportive care and elimination of unabsorbed drug. Haemodialysis and peritoneal dialysis are not effective in significantly reducing serum clarithromycin levels.
Store in a cool, dry place away from light and moisture. After reconstitution, use within 7 days at room temperature or within 14 days if refrigerated. Keep out of reach of children.
Klarix 70 ml may be used in neonates and children at appropriate, physician-recommended doses.
Adults and children over 12 years: Usually 6–14 days
H. pylori eradication in duodenal ulcer patients: 6–14 days as part of combination therapy
Add 45 ml of water to the granules in the bottle and shake well to obtain 70 ml of reconstituted suspension. Each 5 ml of suspension contains 125 mg of clarithromycin.
Clarithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, leading to suppression of bacterial growth and replication.
