4-Quinolone Antibiotic / Fluoroquinolone
Levoxin 750 mg is a synthetic, third-generation fluoroquinolone. It exerts its antibacterial effect by inhibiting two key bacterial enzymes:
Topoisomerase IV – essential for DNA replication and segregation
DNA gyrase – required for DNA supercoiling and repair
By blocking these enzymes, Levoxin prevents bacterial replication, transcription, and repair, leading to bacterial cell death. Its broad-spectrum activity makes it effective against a wide range of respiratory, urinary, and soft tissue pathogens.
Levoxin 750 mg is indicated in adults (18 years and older) for the treatment of:
Respiratory tract infections:
Acute bacterial sinusitis
Acute exacerbations of chronic bronchitis
Community-acquired pneumonia
Nosocomial pneumonia
Skin and soft tissue infections: Uncomplicated and complicated
Chronic bacterial prostatitis
Urinary tract infections: Uncomplicated and complicated (mild to moderate severity)
Acute sinusitis: 500 mg once daily for 10–14 days
Acute exacerbations of chronic bronchitis: 250–500 mg once daily for 7–10 days
Community-acquired pneumonia: 500 mg once or twice daily for 7–14 days
Nosocomial pneumonia: 750 mg once daily for 7–14 days
Skin and soft tissue infections: 500 mg once or twice daily for 7–14 days
Chronic bacterial prostatitis: 500 mg once daily for 28 days
Urinary tract infections: 250 mg once daily for 7–10 days; 3-day regimen may be used for uncomplicated cases
Tablets should be taken orally with a full glass of water. Adequate hydration is recommended to prevent urinary crystallization.

Hypersensitivity to Levoxin, other quinolones, or any components of the formulation
History of tendon disorders related to quinolone therapy
Levoxin 750 mg is generally well tolerated. Common side effects include:
Nausea, vomiting, diarrhea
Abdominal discomfort and flatulence
Rare phototoxicity (0.1%)
Very rare neurological or psychiatric effects may include:
Tremors, anxiety, depression, confusion
Dose adjustment is required for patients with renal impairment
Avoid excessive sun exposure due to potential phototoxicity
Take adequate fluids to prevent concentrated urine and kidney issues
Use with caution in patients with CNS disorders or seizure history
Concurrent use of NSAIDs may increase CNS stimulation risk
Warfarin co-administration may increase bleeding risk
Antacids, iron, and adsorbents: Reduce absorption of Levoxin
NSAIDs: May increase CNS stimulation
Warfarin: May enhance anticoagulant effect and bleeding risk
Levoxin is pregnancy category C. It should only be used if the potential benefit outweighs potential risks to the fetus. Caution is advised during lactation.
Store in a dry place, away from light and heat, at room temperature. Keep out of reach of children.
