Losart 100 mg Tablet contains Losartan Potassium, an Angiotensin II Receptor Blocker (ARB) used primarily for the treatment of hypertension and renal protection in patients with type-2 diabetes and proteinuria. By blocking the effects of angiotensin II, Losart helps relax blood vessels, lower blood pressure, and reduce strain on the heart and kidneys. It may be used alone or in combination with other antihypertensive agents such as thiazide diuretics.

Hypertension:
Losart 100 mg is indicated for the treatment of high blood pressure. It may be used as monotherapy or in combination with other antihypertensive medications (e.g., thiazide diuretics).
Renal Protection in Type-2 Diabetic Patients with Proteinuria:
In hypertensive patients with type-2 diabetes and proteinuria (urine albumin to creatinine ratio >300 mg/g), Losart helps delay the progression of renal disease and reduce the risk of worsening kidney function.
Angiotensin-II Receptor Blocker (ARB)
Losartan is the first orally active, non-peptide angiotensin II receptor blocker. It selectively binds to the AT1 receptor, found in tissues such as the heart, kidneys, adrenal glands, and vascular smooth muscle.
By blocking angiotensin II from binding to AT1 receptors, Losart:
Prevents vasoconstriction
Reduces aldosterone secretion
Promotes vasodilation
Lowers blood pressure
Decreases sodium and water retention
This mechanism contributes to effective blood pressure control and renal protection in diabetic patients.
General Adult Dose:
Usual starting and maintenance dose: 50 mg once daily
If blood pressure is not adequately controlled, dose may be increased up to 100 mg daily (once or divided into two doses).
Volume-depleted patients (e.g., on high-dose diuretics):
Start with 25 mg once daily
Dose Range:
Total daily dose: 25–100 mg
Losart may be taken with or without food.
Rifampicin, Fluconazole: May decrease levels of active metabolite
Hydrochlorothiazide: May enhance antihypertensive effect
Potassium-sparing diuretics (spironolactone, triamterene, amiloride), potassium supplements: Risk of hyperkalemia
NSAIDs (e.g., indomethacin): May reduce antihypertensive effect
Combined use with ACE inhibitors, ARBs, NSAIDs, and diuretics increases risk of renal impairment
Avoid combination with Aliskiren in diabetic patients
Hypersensitivity to Losartan or any component
Pregnancy
Concomitant use with Aliskiren in patients with diabetes
Losart 100 mg is generally well tolerated. Common side effects include:
Dizziness
Diarrhea
Nasal congestion
Cough
Upper respiratory infection
Other possible effects:
Fatigue
Edema
Chest pain
Nausea
Headache
Pharyngitis
Risk of fetal toxicity when used during the second and third trimesters
May cause symptomatic hypotension in volume-depleted patients
Higher plasma levels observed in patients with hepatic impairment
Monitor renal function in patients with kidney disease
Pregnancy Category D.
Use during the second and third trimesters increases the risk of fetal injury or death.
It is unknown whether Losartan is excreted in human milk. A decision should be made whether to discontinue breastfeeding or discontinue the drug, considering its importance to the mother.
Losart 100 mg works by selectively blocking angiotensin II at the AT1 receptor. This prevents vasoconstriction and aldosterone release, resulting in lowered blood pressure, reduced fluid retention, and improved renal protection.
