Losart 50 mg is indicated for:
Hypertension:
Used alone or in combination with other antihypertensive agents (e.g., thiazide diuretics) to control high blood pressure.
Renal Protection in Type-2 Diabetes with Proteinuria:
Recommended in hypertensive patients with type-2 diabetes and proteinuria (urine albumin-to-creatinine ratio >300 mg/g) to delay progression of diabetic nephropathy and reduce the risk of worsening renal disease.
Losartan selectively blocks the AT1 receptor located in vascular smooth muscle, heart, kidneys, and adrenal glands. By inhibiting angiotensin II action, it:
Prevents vasoconstriction
Reduces aldosterone secretion
Decreases sodium and water retention
Lowers systemic vascular resistance
Protects kidney function in diabetic patients
Unlike ACE inhibitors, Losartan does not inhibit bradykinin breakdown, which reduces the risk of persistent cough.
Initial and Maintenance Dose: 50 mg once daily for most patients.
If blood pressure control is inadequate, dose may be increased up to 100 mg daily (either once daily or divided into two doses).
In patients with intravascular volume depletion (e.g., those on high-dose diuretics), a lower starting dose of 25 mg once daily is recommended.
Total daily dose range: 25 mg to 100 mg.
Losart may be taken with or without food.
Rifampicin and Fluconazole: May decrease active metabolite levels.
Potassium-sparing diuretics (e.g., spironolactone), potassium supplements, or potassium-containing salt substitutes: Increase risk of hyperkalemia.
NSAIDs (e.g., indomethacin): May reduce antihypertensive effect and increase risk of renal impairment.
ACE inhibitors or Aliskiren (in diabetic patients): Increased risk of renal dysfunction and hyperkalemia.
Hypersensitivity to Losartan or any component of the formulation
Pregnancy
Concomitant use with Aliskiren in patients with diabetes
Losart 50 mg is generally well tolerated. Common side effects include:
Dizziness
Diarrhea
Nasal congestion
Cough
Upper respiratory tract infection
Less common effects:
Fatigue
Edema
Chest pain
Headache
Nausea
Pharyngitis
Avoid use during pregnancy (especially second and third trimesters) due to risk of fetal injury or death.
Monitor renal function in patients with renal impairment.
Symptomatic hypotension may occur in volume-depleted patients.
Patients with hepatic impairment may have increased plasma concentrations.
Pregnancy Category D: Contraindicated due to risk of fetal harm.
Use during breastfeeding should be carefully evaluated; discontinue either the drug or nursing based on clinical necessity.
Store in a dry place away from heat and light.
Keep out of reach of children.
