Indications

Lubistone 8 mcg is prescribed for:

• Irritable Bowel Syndrome with Constipation (IBS-C): Improves bowel movement frequency and stool consistency

• Opioid-Induced Constipation (OIC): Helps counteract constipation caused by opioid therapy in chronic, non-cancer pain patients

Pharmacology

Lubistone 8 mcg works by activating ClC-2 chloride channels on the apical membrane of the intestinal epithelium. Activation of these channels leads to an efflux of chloride ions into the intestinal lumen, followed by sodium and water, increasing intestinal fluid secretion. This mechanism:

• Enhances intestinal motility and eases stool passage

• Restores mucosal barrier function by supporting tight junction proteins

• Provides targeted action in the gut with minimal systemic absorption

Dosage and Administration

• IBS-C: 8 mcg twice daily with food and water. In severe hepatic impairment, start at 8 mcg once daily and increase if tolerated.

• CIC & OIC: 24 mcg twice daily with food and water. In moderate hepatic impairment, start at 16 mcg twice daily; in severe impairment, start at 8 mcg twice daily.

• Take orally with food and water to reduce nausea and improve tolerance.

Drug Interactions

• Efficacy may be reduced in patients using diphenylheptane opioids.

Contraindications

• Known or suspected mechanical gastrointestinal obstruction

Overdose

Symptoms may include: nausea, diarrhea, vomiting, dizziness, abdominal pain, flushing, dyspnea, pallor, chest discomfort, dry mouth, syncope, and hyperhidrosis. Supportive care is recommended.

Pregnancy and Lactation

• Pregnancy Category C: Use only if the potential benefit justifies the risk

• Excretion in human milk is unknown; caution is advised, and breastfed infants should be monitored for diarrhea
