Perinase 50 mcg/spray is indicated for the relief of inflammatory and pruritic manifestations associated with corticosteroid-responsive dermatoses, including:
Eczema
Dermatitis and other inflammatory skin conditions responsive to corticosteroid therapy
Fluticasone & Combined Preparations (Topical Corticosteroid)
Fluticasone propionate is a synthetic glucocorticoid with high topical anti-inflammatory potency and a low risk of hypothalamic-pituitary-adrenal (HPA) axis suppression following dermal application. This favorable therapeutic index makes it safer for topical use compared to many other potent corticosteroids.
Fluticasone propionate exhibits a high degree of selectivity and strong agonist activity at human glucocorticoid receptors, which are primarily responsible for its anti-inflammatory, antipruritic, and vasoconstrictive effects. In vitro studies demonstrate its strong receptor affinity, while showing weak interaction with progesterone receptors and virtually no affinity for mineralocorticoid, estrogen, or androgen receptors.
The clinical potency of Fluticasone propionate is related to the prolonged half-life of the glucocorticoid-receptor complex, which is approximately 10 hours, allowing sustained anti-inflammatory activity with minimal systemic exposure.
Dosage should be based on the severity of the condition and patient response.
Cream: Apply a thin layer of Fluticasone propionate cream to the affected area once daily.
Ointment: Apply a thin layer of Fluticasone propionate ointment to the affected area twice daily.
Use should be limited to the shortest duration necessary to control symptoms.
Perinase 50 mcg/spray is contraindicated in:
Rosacea
Acne vulgaris
Perioral dermatitis
Primary cutaneous viral infections (e.g., herpes simplex, chicken pox)
Hypersensitivity to Fluticasone propionate or any component of the formulation
Perianal and genital pruritus
It is not indicated for primarily infected skin lesions caused by fungi or bacteria, nor for dermatoses in children under one year of age, including napkin eruptions.

Perinase 50 mcg/spray is generally well tolerated. Reported side effects include:
Local burning sensation
Pruritus
Skin irritation
Prolonged or intensive use of potent corticosteroids may cause local skin changes such as thinning of the skin, striae, dilated superficial blood vessels, hypertrichosis, and hypopigmentation. Secondary infections, allergic contact dermatitis, and worsening of existing dermatoses have also been reported. Extensive or prolonged use, especially under occlusive dressings or in children, may result in systemic absorption leading to features of hypercorticism.
Use during pregnancy should be considered only if the potential benefit to the mother outweighs the possible risk to the fetus. The excretion of Fluticasone propionate in human breast milk has not been established. When used in lactating mothers, the benefits should be weighed carefully against potential risks to the infant.
Caution is advised when applying to large body surface areas or using for prolonged periods, particularly in infants and children, due to the risk of adrenal suppression. Facial skin is more susceptible to atrophic changes with long-term use. In infected inflammatory lesions, appropriate antimicrobial therapy should be initiated, and corticosteroid treatment discontinued if infection spreads.
Acute overdose is unlikely. Chronic overuse or misuse may result in signs of hypercorticism, in which case treatment should be gradually discontinued. Accidental oral ingestion is unlikely to cause toxicity due to very low oral bioavailability.
