Indications

Prosan HZ Tablet is indicated for:

• Hypertension:
  Treatment of high blood pressure in patients who are not adequately controlled on losartan or hydrochlorothiazide alone.

• Reduction of Cardiovascular Risk:
  Effective blood pressure control helps reduce the risk of stroke, heart attack, and other cardiovascular complications.

Pharmacology

Losartan Potassium selectively blocks the binding of angiotensin II to the AT1 receptor found in vascular smooth muscle, adrenal glands, kidneys, and the heart. This inhibition prevents vasoconstriction and reduces aldosterone secretion, resulting in vasodilation and decreased blood pressure.

Hydrochlorothiazide is a thiazide diuretic that acts on the distal renal tubules to inhibit sodium and chloride reabsorption. This leads to increased excretion of sodium and water, reducing plasma volume and peripheral resistance. The combination provides an additive antihypertensive effect greater than either agent alone.

Mode of Action

Prosan HZ Tablet lowers blood pressure by:

• Blocking angiotensin II–mediated vasoconstriction through losartan

• Promoting diuresis and reducing fluid overload through hydrochlorothiazide
  Together, these actions decrease vascular resistance and circulating blood volume, leading to effective and sustained blood pressure reduction.

Dosage and Administration

• Prosan HZ Tablet may be taken with or without food.

• Not recommended as initial therapy except in patients likely to need combination treatment.

Drug Interactions

• Potassium supplements or potassium-sparing diuretics may increase the risk of hyperkalemia.

• NSAIDs may reduce the antihypertensive effect and increase the risk of renal impairment.

• Lithium levels may increase when used concurrently.

• Other antihypertensive agents may enhance blood pressure–lowering effects.

Contraindications

• Hypersensitivity to losartan, hydrochlorothiazide, or sulfonamide-derived drugs

• Pregnancy

• Severe renal impairment or anuria

• Concomitant use with aliskiren in patients with diabetes

Side Effects

Most side effects are mild to moderate and may include:

• Dizziness or lightheadedness

• Fatigue

• Headache

• Electrolyte imbalance

• Upper respiratory tract infection

Less common effects include hypotension, gastrointestinal disturbances, and muscle cramps.

Pregnancy and Lactation

• Pregnancy: Category D. Use during the second and third trimesters may cause fetal harm and should be avoided.

• Lactation: Caution is advised as both components may be excreted in breast milk.
