Telmisartan works by selectively blocking the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and adrenal tissue. Angiotensin II is a potent vasoconstrictor that also stimulates aldosterone secretion, leading to sodium and water retention. By blocking these effects, telmisartan causes vasodilation and reduces peripheral vascular resistance.
Hydrochlorothiazide acts on the distal convoluted tubule of the kidney, inhibiting sodium and chloride reabsorption. This increases the excretion of sodium and water, resulting in a reduction in plasma volume and blood pressure.
The combined action of these two agents produces an additive antihypertensive effect, providing better blood pressure control than either drug alone.

Telma Plus Tablet 40 mg + 12.5 mg is indicated for:
Treatment of essential hypertension in adults
Patients whose blood pressure is not adequately controlled with telmisartan or hydrochlorothiazide alone
Patients requiring combination therapy for effective blood pressure management
Dosage should be individualized based on patient response
The usual recommended dose is one tablet once daily
Tablets may be taken with or without food
Regular monitoring of blood pressure, electrolytes, and renal function is recommended
Not intended for initial therapy unless clearly indicated
Hypersensitivity to telmisartan, hydrochlorothiazide, sulfonamide-derived drugs, or any component of the formulation
Pregnancy and lactation
Severe renal impairment or anuria
Severe hepatic impairment or biliary obstructive disorders
Hypotension may occur, especially in volume- or sodium-depleted patients
Monitor serum electrolytes, particularly potassium, sodium, and magnesium
Use with caution in patients with renal or hepatic impairment
Avoid concomitant use with aliskiren in patients with diabetes
Patients should be advised about adequate hydration
Commonly reported side effects may include:
Dizziness or headache
Hypotension
Electrolyte imbalance
Increased urination
Fatigue
Most adverse effects are mild to moderate and resolve with continued therapy or dose adjustment.
NSAIDs may reduce antihypertensive effect and increase the risk of renal impairment
Potassium supplements or potassium-sparing diuretics may increase the risk of hyperkalemia
Other antihypertensive agents may enhance blood pressure-lowering effects
Telma Plus Tablet is contraindicated during pregnancy due to the risk of fetal harm
Use during breastfeeding is not recommended
Store below 30°C in a cool and dry place
