Amlodipine Besilate inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle cells. This results in relaxation of peripheral arterioles, reduced peripheral vascular resistance, and a subsequent decrease in blood pressure. Amlodipine provides gradual and sustained antihypertensive action, minimizing reflex tachycardia.
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and adrenal tissue. This inhibits vasoconstriction and aldosterone secretion, leading to vasodilation and reduced sodium and water retention.
The combination of amlodipine and telmisartan produces an additive blood pressure-lowering effect, offering superior control compared to either agent alone.

Indications

Telmacal Tablet 5 mg + 40 mg is indicated for:

• Treatment of essential hypertension in adults

• Patients whose blood pressure is not adequately controlled with amlodipine or telmisartan monotherapy

• Patients requiring combination therapy for improved blood pressure control

Dosage and Administration

• Dosage should be individualized based on patient response

• The usual dose is one tablet once daily

• Tablets may be taken with or without food

• Regular monitoring of blood pressure is recommended

• Dose adjustment may be required in elderly patients or those with hepatic impairment

Contraindications

• Hypersensitivity to amlodipine, telmisartan, or any component of the formulation

• Pregnancy and lactation

• Severe hepatic impairment

• Patients with biliary obstructive disorders

Warnings and Precautions
