Tioriva Inhalation Capsule 18 mcg is indicated for:

• Maintenance treatment of Chronic Obstructive Pulmonary Disease (COPD), including chronic bronchitis and emphysema

• Long-term relief of symptoms such as shortness of breath, wheezing, and chest tightness

• Reduction in the frequency of COPD exacerbations

Tiotropium is not indicated for the relief of acute bronchospasm or sudden asthma attacks.

Pharmacology

Tiotropium is a long-acting muscarinic receptor antagonist (LAMA). It works by selectively inhibiting the action of acetylcholine on M3 muscarinic receptors in the airway smooth muscle. This leads to sustained bronchodilation by relaxing the airway muscles and preventing bronchoconstriction. Tiotropium dissociates slowly from the M3 receptor, which allows for once-daily dosing and prolonged symptom control over 24 hours.

Dosage and Administration

• The recommended dose is one inhalation capsule (18 mcg) once daily

• The capsule should be inhaled using a compatible dry powder inhaler device

• Capsules must not be swallowed

• Patients should use the medication at the same time each day for optimal benefit

Contraindications

Tioriva Inhalation Capsule 18 mcg is contraindicated in patients with:

• Known hypersensitivity to Tiotropium, atropine, or its derivatives

• Allergy to any component of the formulation

Side Effects

Common side effects include:

• Dry mouth

• Throat irritation

• Cough after inhalation

• Constipation

• Headache and dizziness

Less common but serious effects may include urinary retention, increased intraocular pressure, or palpitations.

Drug Interactions

• Concomitant use with other anticholinergic medications may increase the risk of adverse effects

• No significant interactions have been reported with commonly used COPD medications such as beta-agonists or inhaled corticosteroids

Pregnancy and Lactation

There are limited data on the use of Tiotropium in pregnant women. It should be used during pregnancy only if the potential benefit outweighs the potential risk. Caution is advised during breastfeeding.

Precautions and Warnings

• Not intended for emergency relief of acute breathing problems

• Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction

• Avoid contact of inhalation powder with eyes
