Tofen 1 mg contains Ketotifen, a potent anti-allergic agent with a strong and long-lasting non-competitive histamine (H1) receptor blocking action. It works by inhibiting the release of allergic mediators such as histamine, leukotrienes, and prostaglandins from mast cells involved in type I allergic reactions.
Additionally, Ketotifen stabilizes mast cells and basophils, reduces eosinophil chemotaxis and degranulation, and inhibits platelet-activating factor (PAF). Through these mechanisms, Tofen 1 mg helps prevent bronchial hyperreactivity and reduces the frequency of asthmatic attacks.

Adults: 1 mg twice daily with food. In severe cases, the dose may be increased to 2 mg twice daily as advised by a physician.
Children above 2 years: 1 mg twice daily with food.
Patients prone to sedation: Start with 0.5–1 mg at night for the first few days.
Elderly: Same as adult dose or as directed by the physician.
Concomitant use with oral antidiabetic agents may cause a reversible reduction in platelet count and should be avoided.
Ketotifen may potentiate the effects of CNS depressants such as alcohol, sedatives, hypnotics, and other antihistamines.
Use with oral antidiabetic drugs is contraindicated due to the risk of reversible thrombocytopenia.

Mild dizziness
These effects are usually transient and subside after a few days of continued therapy.

Although no teratogenic effects have been documented, the use of Ketotifen during pregnancy and lactation is not recommended unless clearly necessary.
Previous asthma treatments should be continued for at least two weeks after starting Tofen 1 mg to avoid asthma exacerbation.
Special caution is required in patients dependent on systemic corticosteroids or ACTH due to possible adrenocortical insufficiency.
Patients should avoid driving or operating machinery during the initial days of treatment until individual response is known.
Alcohol consumption should be avoided as Ketotifen may enhance sedative effects.
In case of intercurrent infection, appropriate antimicrobial therapy should be added.
Symptoms:
Drowsiness, confusion, dyspnea, bradycardia or tachycardia, disorientation, convulsions, severe hypotension, and reversible coma.
Management:
Supportive and symptomatic treatment is recommended.
