Calcium-channel blockers
Verapamil Hydrochloride inhibits the transmembrane influx of calcium ions through slow L-type calcium channels in myocardial cells and vascular smooth muscle during depolarization. Calcium is essential for myocardial contraction and electrical conduction through the atrioventricular (AV) node. By reducing calcium entry, verapamil decreases myocardial contractility, slows AV nodal conduction, and prolongs the refractory period of the AV node. In addition, it relaxes coronary and peripheral vascular smooth muscle, resulting in vasodilation and improved myocardial oxygen supply. These actions make verapamil particularly effective in the acute management of supraventricular arrhythmias and elevated blood pressure.
Veracal Injection 5 mg/2 ml is indicated for the treatment of:
Paroxysmal supraventricular tachycardia
Atrial fibrillation with rapid ventricular response (except in patients with Wolff-Parkinson-White syndrome)
Atrial flutter with rapid conduction
Tachycardias requiring rapid ventricular rate control
Acute hypertension
Acute coronary insufficiency
Prophylaxis and treatment of ectopic arrhythmias, particularly ventricular extrasystoles during halothane anesthesia or in association with adrenaline administration
Veracal Injection should be administered intravenously under strict medical supervision with continuous electrocardiographic monitoring.
Adults:
A dose of 5 mg is given slowly by intravenous injection. If required, a repeat dose may be administered after 5 to 10 minutes. For maintenance of therapeutic effect, intravenous infusion may be used at a rate of 5–10 mg per hour in appropriate infusion solutions. The average total daily dose should not exceed 100 mg.
Pediatric Use:
Newborns: 0.75–1 mg
Infants: 0.75–2 mg
Children 1–5 years: 2–3 mg
Children 6–14 years: 2.5–5 mg
The injection should be administered slowly and only until the desired effect is achieved.

Veracal Injection is contraindicated in patients with:
Severe left ventricular dysfunction
Hypotension or cardiogenic shock
Sick sinus syndrome (unless a functioning pacemaker is present)
Second- or third-degree AV block (without a pacemaker)
Atrial fibrillation or flutter with accessory conduction pathways such as WPW syndrome
Known hypersensitivity to verapamil
Verapamil Injection is generally well tolerated when used appropriately. Possible side effects include hypotension, bradycardia, AV block, dizziness, headache, nausea, flushing, edema, and transient elevation of liver enzymes. Serious adverse effects are more likely with rapid administration or overdose.
Verapamil may cause fetal hypoxia due to maternal hypotension. Intravenous use is not recommended during pregnancy unless the benefits clearly outweigh the risks. Verapamil is excreted in breast milk; therefore, breastfeeding should be discontinued during intravenous treatment.
Caution is required in patients with heart failure, bradycardia, mild AV block, or hepatic impairment. Verapamil may impair the ability to drive or operate machinery and can potentiate the effects of alcohol.
