Indications

Amdocal Tablet 2.5 mg is indicated for:

• Hypertension: Effective in lowering elevated blood pressure in adults, helping to reduce the risk of stroke, myocardial infarction, and other cardiovascular complications.

• Chronic Stable Angina: Reduces the frequency and severity of angina attacks by improving coronary blood flow.

• Variant (Prinzmetal’s) Angina: Relieves chest pain caused by coronary artery spasms.

• Other cardiovascular conditions: May be used in combination with other antihypertensive agents or as part of therapy in patients with left ventricular hypertrophy.

Pharmacology and Mode of Action

Amlodipine is a dihydropyridine calcium channel blocker that inhibits the influx of calcium ions through L-type calcium channels in vascular smooth muscle and cardiac tissue. By preventing calcium entry, amlodipine causes relaxation of arterial smooth muscle, resulting in vasodilation and reduced peripheral vascular resistance. This leads to a decrease in blood pressure and improved myocardial oxygen delivery.

Amlodipine has a gradual onset of action and a long duration, allowing for once-daily dosing and providing steady blood pressure control throughout the day. It selectively targets vascular smooth muscle more than cardiac muscle, minimizing negative effects on heart rate.

Dosage and Administration

• Hypertension: The usual starting dose is 2.5–5 mg once daily. Based on response, the dose may be increased to a maximum of 10 mg once daily.

• Angina: A starting dose of 5 mg once daily is commonly prescribed, which can be adjusted to 10 mg based on symptom control.

Amdocal should be taken orally, with or without food, at the same time each day to maintain consistent blood levels. Tablets should be swallowed whole with water.

Contraindications

Amdocal Tablet 2.5 mg is contraindicated in patients with:

• Known hypersensitivity to amlodipine or any component of the formulation

• Severe hypotension

• Cardiogenic shock

• Obstruction of the outflow tract of the left ventricle, including severe aortic stenosis

Side Effects

Amdocal is generally well tolerated. Common side effects include:

• Peripheral edema (swelling of hands, feet, or ankles)

• Dizziness, headache

• Flushing

• Fatigue

• Palpitations

Rare side effects may include hypotension, hepatic enzyme elevation, gingival hyperplasia, or allergic reactions.

Precautions and Warnings

Patients should be monitored for blood pressure response, particularly during the initial weeks of therapy or dose adjustment. Caution is advised in patients with severe liver impairment, heart failure, or unstable angina. Abrupt discontinuation should be avoided to prevent exacerbation of angina.

Pregnancy and Lactation
