Arlin 600 mg is a synthetic antibiotic from the oxazolidinone class, designed to treat severe infections caused by Gram-positive bacteria, including strains resistant to other antibiotics. It is available as oral tablets, oral suspension, and intravenous infusion, making it versatile for both inpatient and outpatient therapy.

Arlin 600 mg works by inhibiting bacterial protein synthesis through a mechanism distinct from other antibiotics. It binds to the 23S rRNA of the 50S ribosomal subunit, preventing formation of the functional 70S initiation complex, which is essential for bacterial translation. This mechanism results in bacteriostatic activity against enterococci and staphylococci, and bactericidal activity against most streptococcal strains.

Indications

Arlin 600 mg is indicated for the treatment of:

• Vancomycin-Resistant Enterococcus faecium (VRE) infections, including cases with concurrent bacteremia

• Nosocomial pneumonia caused by Staphylococcus aureus (MSSA & MRSA) or Streptococcus pneumoniae (including multi-drug resistant strains)

• Complicated skin and skin structure infections, including diabetic foot infections (without osteomyelitis) caused by S. aureus, S. pyogenes, or S. agalactiae

• Uncomplicated skin and skin structure infections caused by S. aureus (MSSA) or S. pyogenes

• Community-acquired pneumonia caused by S. pneumoniae (including multi-drug resistant strains) or S. aureus (MSSA), including cases with concurrent bacteremia

• Combination therapy may be used if Gram-negative organisms are suspected

Dosage & Administration

Adults and Adolescents (≥12 years):

• Complicated skin/skin structure infections or community-acquired pneumonia: 600 mg IV or oral twice daily for 10–14 days

• Nosocomial pneumonia / VRE infections: 600 mg IV or oral twice daily for 14–28 days

• Uncomplicated skin infections: 400–600 mg oral once or twice daily for 10–14 days

Pediatrics (Birth–11 years):

• <5 yrs: 10 mg/kg oral three times daily

• 5–11 yrs: 10 mg/kg oral twice daily

• IV dosing: same as oral, adjusted by age/weight

Neonates (<7 days):

• Preterm neonates <34 weeks gestation: 10 mg/kg every 12 hours

• By 7 days of life: 10 mg/kg three times daily

Administration:

• Oral suspension: Reconstitute with boiled, cooled water; use within 21 days

• IV infusion: Administer over 30–120 minutes; single-use infusion bags; avoid additives

Interactions

• Monoamine oxidase inhibitor (MAOI) activity: May enhance pressor response to adrenergic agents

• Serotonergic agents: Risk of serotonin syndrome; monitor patients receiving SSRIs, triptans, tricyclics, or serotonergic drugs

• Other: Use caution with vasopressors, dopaminergic agents, meperidine, buspirone

Contraindications

• Hypersensitivity to Arlin 600 mg or components

• Use with MAOIs or within 2 weeks of discontinuation

• Patients with uncontrolled hypertension, pheochromocytoma, thyrotoxicosis, or carcinoid syndrome

• Avoid concomitant use with certain adrenergic and serotonergic agents

Side Effects

Most adverse effects are mild to moderate:

• Diarrhea, nausea, headache

• Oral or vaginal moniliasis

• Hypertension, dyspepsia, localized abdominal pain

• Pruritus, tongue discoloration

Pregnancy & Lactation

• Pregnancy Category C: Use only if benefit outweighs risk

• Unknown excretion in human milk; exercise caution when nursing

Precautions & Warnings

• Monitor for serotonin syndrome with concomitant serotonergic drugs

• Evaluate for visual changes, convulsions, nausea, vomiting, or unexplained acidosis

• Adjust dosing in neonates and children based on weight and age

Overdose

• Symptomatic and supportive care recommended

• ~30% removed by 3 hours of hemodialysis

• No specific antidote

Storage

• Store at 15–30°C, away from light and moisture

• Keep out of reach of children

• Oral suspension: Use within 21 days after reconstitution.
