Epleron 25 mg is a prescription medication indicated for the management of congestive heart failure following an acute myocardial infarction and for hypertension. Mechanism of Action:
Epleron 25 mg selectively binds to mineralocorticoid receptors, inhibiting the action of aldosterone. By blocking aldosterone in epithelial (kidney, GI tract) and nonepithelial (heart, blood vessels, brain) tissues, Epleron reduces sodium reabsorption, prevents water retention, and mitigates vascular remodeling and endothelial dysfunction, ultimately helping to control blood pressure and protect the cardiovascular system.
Indications:
Congestive heart failure after acute myocardial infarction
Hypertension
Dosage & Administration:
Congestive Heart Failure: Initiate at 25 mg once daily, titrating to 50 mg once daily over 4 weeks as tolerated.
Hypertension: Start at 50 mg once daily. If necessary, increase to 50 mg twice daily. Higher doses are not recommended due to risk of hyperkalemia.
Can be taken with or without food.
Contraindications:
Hyperkalemia
Severe renal or hepatic impairment
Concurrent use with potent CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin)
Use with other potassium-sparing diuretics
Precautions & Warnings:
Use cautiously in patients with hyperkalemia, severe kidney disease, or diabetes with proteinuria.
Monitor electrolytes, especially potassium, during therapy.
Potential Drug Interactions:
Increased risk of hyperkalemia with ACE inhibitors, ARBs, ciclosporin, tacrolimus, or trimethoprim
Reduced antihypertensive effect with NSAIDs or glucocorticoids
Enhanced hypotensive effect with α1-blockers, TCAs, or other hypotensive agents
Increased plasma levels with CYP3A4 inhibitors (e.g., fluconazole, verapamil)
Side Effects:
Common: headache, dizziness, diarrhea, stomach pain, nausea, cough, or flu-like symptoms.
Serious: rash, itching, swelling, severe dizziness, or difficulty breathing (seek immediate medical attention).
Pregnancy & Lactation:
Pregnancy: Use only if potential benefit outweighs risk.
Lactation: Unknown excretion in human milk; weigh the benefits of therapy against potential risks to the infant.
Special Populations:
Pediatric: Safety and efficacy not established
Geriatric: No significant differences in efficacy or safety observed
Overdose:
Symptoms may include hyperkalemia and hypotension. Management includes symptomatic treatment, activated charcoal if appropriate, and standard hyperkalemia therapy.
Storage:
Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children.
