Esipram 5 mg is indicated in the treatment of:
Depressive illness
Generalized anxiety disorder
Obsessive-compulsive disorder
Social anxiety disorder
SSRIs & related anti-depressant drugs
Esipram 5 mg acts as a selective inhibitor of serotonin (5-hydroxytryptamine, 5-HT) reuptake in the central nervous system. By blocking the reuptake of serotonin into presynaptic neurons, it increases serotonin availability in the synaptic cleft, enhancing serotonergic neurotransmission. This mechanism is responsible for its antidepressant and anxiolytic effects. Esipram 5 mg has minimal or no affinity for other receptors, including dopamine, adrenergic, histaminergic, muscarinic, benzodiazepine, and opioid receptors, which contributes to its lower incidence of unwanted side effects compared to older antidepressants.
Adults: The usual initial dose is 10 mg once daily. Based on clinical response and tolerability, the dose may be increased to a maximum of 20 mg daily after at least one week.
Panic disorder: Adults may start with 5 mg once daily, increased to 10 mg after 7 days. The maximum dose is 20 mg daily.
Social anxiety disorder: Adults typically start at 10 mg once daily. Dose adjustments may be made after 2–4 weeks, with a usual range of 5–20 mg daily.
Elderly: A recommended dose of 10 mg once daily is generally sufficient. Lower maintenance doses may be appropriate.
Esipram 5 mg should be taken orally once daily, with or without food, as directed by a physician.
Concomitant use with monoamine oxidase inhibitors (MAOIs) is contraindicated
Esipram 5 mg should not be started until at least 2 weeks after stopping an MAOI
MAOIs should not be initiated until at least one week after discontinuing Esipram 5 mg
Esipram 5 mg is contraindicated in patients with:
Concomitant use of MAOIs
Concomitant use of pimozide
Known hypersensitivity to escitalopram, citalopram, or any excipients
Esipram 5 mg is generally well tolerated. Common side effects may include:
Nausea
Insomnia or drowsiness
Increased sweating
Fatigue
Sexual dysfunction, including problems with ejaculation
Most side effects are mild, transient, and tend to resolve with continued treatment.
Pregnancy: Safety during pregnancy has not been fully established. Use only if the potential benefit justifies the potential risk to the fetus.
Lactation: Esipram 5 mg is excreted in breast milk. Use during breastfeeding only if clearly necessary and advised by a physician.
Abrupt discontinuation may lead to withdrawal symptoms such as dizziness, irritability, anxiety, headache, insomnia, and sensory disturbances. Gradual dose reduction is recommended when discontinuing therapy. Patients should be monitored closely during dose changes or discontinuation.
