Hypovolemia, a condition of decreased blood volume, may result from trauma, surgery, burns, or severe fluid loss. Hydroxyethyl Starch, a synthetic colloid, is a macromolecular substance that remains in the vascular compartment longer than crystalloids, providing sustained plasma volume expansion. Sodium chloride (normal saline) balances electrolytes and ensures isotonicity with blood plasma.

Expansol Injection 6%+0.9% is indicated for:
Acute hypovolemia caused by hemorrhage, trauma, or surgery
Shock associated with fluid loss
Burns where plasma volume expansion is necessary
Prevention of hypotension during major surgical procedures
Adjunctive fluid therapy when crystalloids alone are insufficient to restore intravascular volume
Adult Dosage:
Initial dose: 500–1000 ml intravenously, administered slowly, depending on the patient’s condition and clinical response
Maintenance infusion: Adjusted according to hemodynamic parameters, ongoing blood loss, and patient tolerance
Maximum daily dose of HES: 20–25 ml/kg body weight for critically ill patients, and up to 50 ml/kg in perioperative patients
Administration Guidelines:
Administer via a sterile intravenous infusion set under aseptic conditions
Use a central or peripheral vein depending on infusion rate and patient condition
Monitor blood pressure, heart rate, and urine output to assess the effectiveness of therapy
Avoid rapid administration in patients at risk of fluid overload, such as those with cardiac or renal insufficiency
Do not mix with other medications in the same infusion line unless compatibility is confirmed
Hydroxyethyl Starch in Expansol increases the colloid osmotic pressure in the plasma, drawing water from the interstitial space into the intravascular compartment. This restores circulating blood volume, improves cardiac output, and maintains adequate tissue perfusion. Sodium chloride provides isotonicity and prevents electrolyte imbalance during fluid replacement therapy.

Expansol Injection should not be used in patients with:
Severe renal impairment or oliguria
Uncontrolled bleeding due to coagulation disorders
Hypersensitivity to hydroxyethyl starch or any component of the formulation
Severe heart failure or pulmonary edema where fluid overload is a risk
Possible adverse reactions may include:
Mild allergic reactions (rash, itching, or urticaria)
Fluid overload leading to pulmonary edema in susceptible patients
Temporary increase in blood pressure
Rare: coagulopathy with prolonged or high-dose use
Local reactions at the infusion site such as redness or pain
Monitor renal function during therapy, especially in critically ill patients
Use cautiously in elderly patients or those with cardiovascular compromise
Avoid exceeding recommended dosage to prevent coagulation disturbances or volume overload
Stop infusion immediately if signs of allergic reaction or pulmonary edema appear
Safety in pregnancy and lactation has not been firmly established. Use only if clearly indicated and with close monitoring.
