Pharmacology

Olopatadine, the active ingredient in Lopadine DS 0.2%, works through:

• H1 Histamine Receptor Antagonism: Blocks histamine-mediated allergic reactions in the conjunctiva.

• Mast Cell Stabilization: Prevents the release of histamine and other inflammatory mediators from mast cells, reducing allergic response.

Olopatadine is highly selective for H1 receptors and does not interfere with alpha-adrenergic, dopamine, or muscarinic receptors, which reduces systemic side effects. After ocular application, systemic absorption is minimal.

Indications

Lopadine DS 0.2% is indicated for:

• Relief of ocular itching associated with allergic conjunctivitis

• Treatment of seasonal and perennial eye allergies

Dosage and Administration

• 0.2% Sterile Eye Drops: One drop in the affected eye once daily

• Other strengths (for context):

  • 0.1%: One drop twice daily

  • 0.7%: One drop once daily

Special Instructions:

• Remove soft contact lenses before use.

• Wait at least 10 minutes after instillation before reinserting contact lenses.

Interaction

• Additive CNS depression may occur if used with other CNS depressants.

Contraindications

• Known hypersensitivity to Olopatadine or any component of the ophthalmic solution.

Side Effects

Common and mild side effects include:

• Headache (~7% incidence)

• Blurred vision

• Burning or stinging sensation

• Dry eyes, pruritus, hyperemia

• Less common: asthenia, nausea, pharyngitis, sinusitis, lid edema, taste perversion

Pregnancy and Lactation

• Should be used in pregnancy only if clearly needed.

• Caution in nursing mothers; systemic absorption is minimal but caution is advised.

Precautions and Warnings

• Not indicated for contact lens-related irritation.

• Avoid contaminating the dropper tip.

• Do not use beyond 30 days after first opening.

Overdose

• Symptoms: Drowsiness in adults; initial agitation followed by drowsiness in children.

• Management: Symptomatic and supportive treatment.

Storage Conditions

• Store below 30°C in a cool, dry place, protected from light.

• Keep out of reach of children.

Use in Special Populations

• Geriatric Use: No significant differences in safety or effectiveness compared to younger adults.
