Indications

Maxima 20 mg is indicated for:

• Symptomatic relief of GERD (Gastroesophageal Reflux Disease)

• Healing of erosive esophagitis

• Maintenance of healed erosive esophagitis

• Eradication of Helicobacter pylori infection (in combination with amoxicillin and clarithromycin) in patients with duodenal ulcer

• Zollinger-Ellison syndrome

• Treatment of duodenal and gastric ulcers

• Acid-related dyspepsia

Mode of Action

Esomeprazole is the S-isomer of omeprazole and works by selectively inhibiting the H+/K+-ATPase enzyme system (proton pump) in gastric parietal cells. This enzyme is responsible for the final step in gastric acid production.

By blocking this pump, Maxima 20 mg:

• Significantly reduces gastric acid secretion

• Promotes ulcer healing

• Relieves acid-related symptoms

Dosage & Administration

General Administration:

• Take one tablet at least 1 hour before a meal.

• Swallow whole; do not crush or chew.

Recommended Dosage:

• Healing of erosive esophagitis: 20–40 mg once daily for 4–8 weeks

• Maintenance therapy: 20 mg once daily

• Symptomatic GERD: 20 mg once daily for 4 weeks

• Duodenal ulcer: 20 mg once daily for 2–4 weeks

• Gastric ulcer: 20–40 mg once daily for 4–8 weeks

• H. pylori eradication (Triple therapy):

  • Esomeprazole 40 mg once daily

  • Amoxicillin 1000 mg twice daily

  • Clarithromycin 500 mg twice daily

  • Duration: 10 days

• Zollinger-Ellison syndrome: 20–80 mg once daily (individualized)

Intravenous formulation may be used when oral therapy is not feasible.

Drug Interactions

Caution is advised when used with:

• Diazepam (may increase plasma levels)

• Drugs affected by gastric pH (e.g., ketoconazole, iron salts, digoxin)

• Warfarin (monitor INR)

• Clarithromycin (increases esomeprazole levels)

Contraindications

• Hypersensitivity to esomeprazole or other proton pump inhibitors

Side Effects
