Pantobex 20 mg is a proton pump inhibitor (PPI) containing Pantoprazole, used to reduce gastric acid secretion in a wide range of acid-related gastrointestinal disorders. Indications

Pantobex 20 mg is recommended in conditions where inhibition of gastric acid secretion is beneficial, including:

• Gastric (stomach) ulcer

• Gastroesophageal reflux disease (GERD)

• Duodenal ulcer

• Non-steroidal anti-inflammatory drug (NSAID)–induced gastric or duodenal ulcer

• Helicobacter pylori eradication (in combination with appropriate antibiotics)

• Zollinger–Ellison syndrome and other pathological hypersecretory conditions

Pharmacology

Pantoprazole, the active ingredient of Pantobex 20 mg, selectively and irreversibly inhibits the H⁺/K⁺-ATPase enzyme system (proton pump) located on the gastric parietal cell membrane. This action blocks the final step of acid production in the stomach. As a result, Pantobex 20 mg suppresses both basal and stimulated gastric acid secretion for more than 24 hours, regardless of the type of stimulus. This sustained acid suppression creates an optimal environment for mucosal healing and symptom relief.

Dosage and Administration

Oral Administration:

• Benign gastric ulcer: 40 mg once daily in the morning for 4 weeks; may continue for another 4 weeks if needed

• Gastroesophageal reflux disease: 20–40 mg once daily for 4 weeks; maintenance dose 20 mg daily

• Duodenal ulcer: 40 mg once daily for 2 weeks; extend if required

• H. pylori eradication: 40 mg twice daily with antibiotics for one week

• NSAID-associated ulcer prophylaxis: 20 mg once daily

• Zollinger–Ellison syndrome: Initial dose 80 mg daily, adjusted based on response

Intravenous Administration:
Pantobex 20 mg IV is used when oral therapy is not possible, following standard dosing protocols for ulcers, GERD with erosive esophagitis, prevention of rebleeding, acid aspiration prophylaxis, and hypersecretory conditions.

Drug Interactions

Clinical studies have shown no significant drug interactions with Pantobex 20 mg.

Contraindications

Pantobex 20 mg is contraindicated in patients with known hypersensitivity to Pantoprazole or any component of the formulation.

Side Effects

Pantobex 20 mg is generally well tolerated. Common adverse effects include headache and diarrhea. Rarely, abdominal pain, flatulence, rash, insomnia, and mild metabolic changes may occur.

Pregnancy and Lactation

Pantobex 20 mg is classified as a Pregnancy Category B drug. It should be used during pregnancy only if clearly needed. As Pantoprazole is excreted in breast milk, a decision should be made whether to discontinue breastfeeding or the medication.

Precautions and Warnings

Tablets should not be crushed, chewed, or split. Long-term use may be associated with vitamin B12 deficiency or increased risk of osteoporosis-related fractures.
