Vilanterol Trifenatate is a highly selective, long-acting beta₂-adrenergic receptor agonist. It produces sustained relaxation of bronchial smooth muscle by increasing intracellular cyclic AMP levels, leading to prolonged bronchodilation and improved airflow.

Umeclidinium Bromide is a long-acting muscarinic antagonist that blocks acetylcholine-mediated bronchoconstriction by inhibiting M₃ receptors in airway smooth muscle. This results in sustained bronchodilation and reduced airway resistance.

Fluticasone Furoate is a potent inhaled corticosteroid with strong anti-inflammatory properties. It reduces airway inflammation by inhibiting inflammatory cell activity, cytokine production, and mucosal edema. This helps decrease airway hyperresponsiveness and lowers the frequency of COPD exacerbations.

The synergistic action of these three agents offers superior symptom control and lung function improvement compared to mono or dual therapy.

Indications

Trilexa Inhalation Capsule is indicated for:

• Maintenance treatment of chronic obstructive pulmonary disease (COPD)

• Management of moderate to severe COPD requiring triple inhaler therapy

• Reduction of COPD exacerbations in patients with a history of frequent flare-ups

This medication is not intended for the relief of acute bronchospasm.

Dosage & Administration

• The recommended dose is one inhalation capsule once daily, using a suitable dry powder inhaler device, or as directed by the physician.

• Capsules are for inhalation use only and must not be swallowed.

• Regular daily use is essential to achieve maximum therapeutic benefit.

• Patients should be advised to rinse their mouth with water after inhalation to minimize the risk of oral candidiasis.

Dosage should be individualized based on disease severity and clinical response.

Contraindications

• Known hypersensitivity to Vilanterol, Fluticasone Furoate, Umeclidinium, or any component of the formulation

• Use as primary therapy for acute asthma attacks or severe bronchospasm

Side Effects

Trilexa Inhalation Capsule is generally well tolerated. Possible side effects include:

• Headache and dizziness

• Dry mouth or throat irritation

• Cough or hoarseness

• Palpitations or tremor

• Oral fungal infections with prolonged corticosteroid use

Most adverse effects are mild and transient.

Precautions & Warnings

• Use with caution in patients with cardiovascular disorders, diabetes, glaucoma, or urinary retention

• Inhaled corticosteroids may increase the risk of pneumonia in COPD patients

• Avoid exceeding the prescribed dose

• Not suitable as a rescue medication for sudden breathing difficulty
