Trocer 400 mcg/Spray is a fast-acting sublingual nitrate preparation used for the acute relief and short-term prevention of angina pectoris associated with coronary artery disease. It works rapidly by dilating coronary blood vessels, improving blood flow to the heart muscle, and reducing cardiac workload.

Trocer 400 mcg/Spray releases free radical nitric oxide (NO), which activates guanylate cyclase. This leads to an increased level of guanosine 3′,5′-monophosphate (cGMP) in smooth muscle and other tissues. The rise in cGMP causes dephosphorylation of myosin light chains, resulting in relaxation of vascular smooth muscle and vasodilatation. These effects help reduce myocardial oxygen demand and relieve anginal pain.

One sublingual dose should be administered at the first sign of an acute anginal attack.
The dose may be repeated every five minutes until relief is achieved.
If pain persists after a total of three doses within 15 minutes, immediate medical attention is advised.
Concomitant use with antihypertensive agents, beta-adrenergic blockers, phenothiazines, or other nitrates may enhance hypotensive effects.
Marked orthostatic hypotension has been reported with concurrent use of calcium channel blockers and organic nitrates.
Alcohol may potentiate hypotension when used with Trocer 400 mcg/Spray.
Aspirin may enhance the vasodilatory and hemodynamic effects.
Use with ergotamine or related drugs should be avoided or closely monitored due to the risk of ergotism.
Early myocardial infarction
Severe anemia
Increased intracranial pressure
Known hypersensitivity to Trocer 400 mcg/Spray
Concomitant use with sildenafil citrate or other phosphodiesterase-5 inhibitors due to the risk of severe hypotension
Headache, which may be severe and persistent
Vertigo

Palpitations
Occasional postural hypotension
Trocer 400 mcg/Spray should be used during pregnancy only if clearly needed.
It is not known whether the drug is excreted in human breast milk; caution is advised during lactation.
Use the lowest effective dose to control acute anginal attacks.
Excessive or frequent use may lead to tolerance.
Use with caution in patients who are volume-depleted or already hypotensive.
Store in a cool, dry place away from light and heat.
Keep out of reach of children.
