It is also indicated for prophylaxis of febrile convulsions and post-traumatic epilepsy, as well as bipolar disorder and migraine prevention.

Primary Anti-Epileptic Drugs

All types of epilepsy:
Partial seizures
Generalized tonic-clonic seizures (grand mal)
Absence seizures (petit mal)
Myoclonic and atonic seizures
Mixed seizures including absence attacks
Prophylaxis of febrile convulsions
Prophylaxis of post-traumatic epilepsy
Bipolar disorder (mood stabilization)
Prophylaxis of migraine
Sodium Valproate in Valex CR Tablet increases GABA concentrations in the central nervous system, enhancing inhibitory neurotransmission and stabilizing neuronal activity. This mechanism reduces seizure frequency and intensity.
Pharmacokinetics of CR formulation:
Slow and sustained absorption, providing stable plasma levels
Reduced peak-trough fluctuations compared to immediate-release tablets
Metabolized in the liver; partially excreted in urine
Half-life: 9–16 hours, allowing once- or twice-daily dosing
Adults:
Initial dose: 600 mg/day in divided doses (preferably after food)
Titration: Increase by 200 mg/day at 3-day intervals as required
Maintenance dose: 1–2 g/day, divided doses (20–30 mg/kg/day)
Children (up to 20 kg):
Initial dose: 20 mg/kg/day, divided doses
Titrated according to clinical response and plasma levels
Children (over 20 kg):
Initial dose: 400 mg/day, increased to 20–30 mg/kg/day as needed
Bipolar disorder:
Initially 600 mg/day, maintenance 1–2 g/day in divided doses
Migraine prophylaxis:
400–600 mg/day, may require up to 1–1.5 g/day
Important:
Swallow tablets whole; do not crush or chew
Administer after food to minimize gastrointestinal discomfort
Hypersensitivity to Sodium Valproate
Liver dysfunction or severe hepatic impairment

Common side effects:

Crosses the placenta; exposure in the first trimester may lead to neural tube defects
Excreted in breast milk; caution advised
