Tenofovir is an acyclic nucleotide analogue of adenosine monophosphate. Xynovir 300 mg is a prodrug that, after oral administration, is converted into its active form, tenofovir diphosphate, inside the body. This active metabolite inhibits hepatitis B virus polymerase and HIV reverse transcriptase by competing with natural deoxyribonucleotide substrates. Once incorporated into viral DNA, it causes premature DNA chain termination, thereby preventing viral replication. Its dual activity against HIV and hepatitis B makes it a valuable option in co-infected patients.
Xynovir 300 mg is indicated for the treatment of:
Chronic hepatitis B virus infection in adults
HIV-1 infection in adults, used in combination with other antiretroviral agents
The medicine helps reduce viral load, improve immune function in HIV-infected patients, and decrease liver inflammation and disease progression in hepatitis B.
For adults aged 18 years and above with adequate renal function, the recommended dose is:
300 mg once daily, taken with or without food
Dose adjustment is required in patients with renal impairment. In patients undergoing hemodialysis, 300 mg is recommended once every 7 days or after a cumulative 12 hours of dialysis. Treatment should be taken regularly as prescribed to maintain effective viral suppression.

Co-administration of Tenofovir with antiretroviral agents, entecavir, lamivudine, methadone, oral contraceptives, ribavirin, and tacrolimus has not shown clinically significant drug interactions. However, caution is advised when used with drugs that are primarily eliminated by the kidneys or known to affect renal function, as combined use may increase the risk of nephrotoxicity.
Xynovir 300 mg is contraindicated in patients with known hypersensitivity to Tenofovir or any component of the formulation.
Xynovir is generally well tolerated. The most commonly reported side effects include:
Nausea
Vomiting
Diarrhea
Flatulence
These effects are usually mild and transient.
Pregnancy: Category B. Xynovir should be used during pregnancy only if clearly needed and prescribed by a physician.
Lactation: It is not known whether Tenofovir is excreted in human milk. Mothers are advised not to breastfeed while taking this medication.
Tenofovir should not be co-administered with products containing emtricitabine and tenofovir or with adefovir dipivoxil. Although rare, lactic acidosis and severe hepatomegaly with steatosis have been reported; treatment should be discontinued if such conditions develop. Severe acute exacerbation of hepatitis may occur after stopping therapy, so close monitoring of liver function is recommended.
Pediatric: Safety and effectiveness in patients below 18 years have not been established.
Geriatric: Dose selection should be cautious, with monitoring of renal function.
Hepatic impairment: No dose adjustment is required.
