Pharmacology

Zolium XR 2 mg binds with high affinity to the GABA-benzodiazepine receptor complex. By enhancing the effect of GABA, the primary inhibitory neurotransmitter, it increases chloride ion influx into neurons, hyperpolarizing the cell membrane and reducing excessive neuronal excitation. It acts on BNZ1 (sleep mediation) and BNZ2 (muscle relaxation, anticonvulsant activity, motor coordination, and memory) receptors, producing anxiolytic, sedative, and muscle-relaxant effects.

Dosage and Administration

Adults:

• Initial dose: 0.25 mg 2–3 times daily

• Dose may be increased gradually in 0.25 mg increments based on clinical response

• Maximum usual dose: 4 mg/day; some panic disorder cases may require up to 10 mg/day under careful supervision

Elderly or Debilitated Patients:

• Initial dose: 0.125 mg 2–3 times daily

• Gradual increase according to tolerance

Administration Notes:

• Extended-release tablets should be taken intact; do not chew, crush, or break

• Discontinuation should be gradual (reduce by 0.5 mg every 3 days) to avoid withdrawal effects

Drug Interactions

• CNS depressants, other psychotropic medications, anticonvulsants, antihistamines, alcohol, and oral contraceptives may potentiate sedation and drowsiness

Contraindications

Overdose

Symptoms:

• Somnolence, confusion, impaired coordination, diminished reflexes, coma

Management:

• Supportive care and gastric lavage if ingestion is recent

• Monitor respiratory and cardiovascular function

Pregnancy and Lactation

• Category D: Avoid during pregnancy

• Likely excreted in breast milk; nursing not recommended
