Amdocal 10 mg is a long-acting calcium channel blocker used for the management of hypertension and coronary artery disease. It can be used alone or in combination with other antihypertensive or antianginal agents. Amdocal is effective in treating conditions such as chronic stable angina, vasospastic angina, and angiographically documented coronary artery disease in patients without heart failure or significantly reduced ejection fraction. Its dual action on the cardiovascular system helps reduce blood pressure, decrease myocardial oxygen demand, and improve blood flow to the heart.

Amdocal 10 mg is indicated for:
Hypertension (high blood pressure)
Coronary artery disease, including:
Chronic stable angina
Vasospastic (variant) angina
Angiographically documented coronary artery disease in patients without heart failure or ejection fraction <40%
Calcium Channel Blockers
Amdocal 10 mg works by inhibiting the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This results in:
Vasodilation of peripheral arteries, reducing peripheral vascular resistance and afterload
Decreased cardiac contractility, lowering myocardial oxygen demand
Coronary artery dilation, improving oxygen delivery to ischemic heart tissue
The antihypertensive effect is primarily due to direct relaxation of vascular smooth muscle, while the anti-anginal effect arises from a combination of reduced cardiac workload and improved myocardial perfusion.
Adults:
Starting dose: 5 mg once daily
Maximum dose: 10 mg once daily
Pediatric (6–17 years):
Starting dose: 2.5 mg once daily, up-titrated to 5 mg if blood pressure goal is not achieved after 4 weeks
Children under 6 years: safety and efficacy not established
Special Populations:
Elderly: normal adult dosage recommended; titrate carefully
Hepatic impairment: start at 2.5 mg once daily and titrate slowly
Renal impairment: no dose adjustment needed
Amdocal 10 mg can be safely co-administered with:
Thiazide diuretics, alpha blockers, beta blockers
ACE inhibitors, long-acting nitrates, sublingual nitroglycerin
NSAIDs, antibiotics, oral hypoglycemic agents
Note: Grapefruit or grapefruit juice may increase bioavailability, enhancing blood pressure-lowering effects.

Known hypersensitivity to Amdocal 10 mg or any component of the formulation
Common side effects include:
Headache
Edema (dose-related)
Fatigue, nausea, abdominal pain, somnolence
Rare but serious effects may occur in sensitive patients or with overdose.
Symptomatic hypotension is possible, particularly in severe aortic stenosis
Worsening angina or acute myocardial infarction may develop after initiating or increasing dosage in severe coronary artery disease
Titrate slowly in patients with hepatic impairment
Symptoms: Excessive vasodilation, reflex tachycardia, hypotension, shock
Management: Active cardiovascular support, IV fluids, elevation of extremities, intravenous calcium gluconate, gastric lavage if necessary, and monitoring of cardiac and respiratory function
Pregnancy: Category C; use only if benefit outweighs risk
Lactation: Excretion in breast milk unknown; consider discontinuing nursing
