Prosan 50 mg is indicated for the treatment and management of the following conditions:
Hypertension: Used alone or in combination with other antihypertensive agents such as thiazide diuretics to lower elevated blood pressure.
Renal Protection in Type 2 Diabetes: Recommended for hypertensive type 2 diabetic patients with proteinuria (urine albumin-to-creatinine ratio greater than 300 mg/g) to delay the progression of renal disease.
Angiotensin II Receptor Blocker (ARB)
Prosan 50 mg contains losartan, the first orally active non-peptide angiotensin II receptor blocker. It selectively binds to the AT1 receptor found in various tissues including the heart, kidneys, adrenal glands, and vascular smooth muscle. By blocking the effects of angiotensin II, it prevents vasoconstriction and reduces aldosterone secretion, leading to lowered blood pressure and decreased strain on the cardiovascular system.
The usual starting and maintenance dose for most patients is 50 mg once daily.
If blood pressure control is inadequate, the dose may be adjusted to 25 mg twice daily or increased as needed.
Patients with intravascular volume depletion, such as those receiving high-dose diuretics, should consider a lower starting dose of 25 mg once daily.
Prosan 50 mg may be administered once or twice daily.
The total daily dose generally ranges from 25 mg to 100 mg, depending on patient response and clinical condition.
Rifampicin and fluconazole may reduce levels of the active metabolite of Prosan 50 mg.
Concomitant use with hydrochlorothiazide may enhance antihypertensive effects.
Use with potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes may increase serum potassium levels.
Non-steroidal anti-inflammatory drugs such as indomethacin may reduce the antihypertensive effect.
Combined use with ACE inhibitors, ARBs, NSAIDs, and thiazide diuretics may increase the risk of renal impairment.
Hypersensitivity to any component of the formulation.
Pregnancy.
Concomitant use with aliskiren in patients with diabetes mellitus.
Prosan 50 mg is generally well tolerated. Most side effects are mild and transient. Commonly reported adverse effects include:
Dizziness
Diarrhea
Nasal congestion
Cough
Upper respiratory tract infections
Less common effects may include fatigue, edema, chest pain, nausea, headache, and pharyngitis.
Prosan 50 mg is classified as Pregnancy Category D. Use during the second and third trimesters can cause fetal harm, including impaired renal function and increased fetal and neonatal morbidity and mortality.
As it is not known whether Prosan 50 mg is excreted in human milk, a decision should be made to discontinue either breastfeeding or the medication, considering the importance of therapy to the mother.
Use during pregnancy should be avoided due to serious fetal risks.
Symptomatic hypotension may occur in volume-depleted patients.
Patients with hepatic impairment, especially cirrhosis, may experience increased plasma levels of the drug.
Renal function should be monitored, particularly in patients with pre-existing renal impairment, as changes including renal failure have been reported.

Prosan 50 mg blocks the binding of angiotensin II to AT1 receptors, thereby inhibiting vasoconstriction and aldosterone release. This results in reduced blood pressure, improved renal blood flow, and protection against hypertensive and diabetic renal complications.
