Twindopa Tablet is a combination antiparkinson medicine containing Levodopa and Carbidopa, designed to improve motor function and relieve the core symptoms of Parkinson’s disease and related syndromes. By increasing dopamine levels in the brain, Twindopa Tablet helps patients regain better control of movement and daily activities.

Twindopa Tablet is indicated for:
Treatment of Parkinson’s disease and Parkinsonian syndromes
Relief of rigidity and bradykinesia
Management of tremor, dysphagia, sialorrhea, and postural instability associated with Parkinson’s disease
Improvement of motor recovery when Levodopa plus Carbidopa is administered before physiotherapy in post-stroke patients
Levodopa is the metabolic precursor of dopamine. It crosses the blood–brain barrier and is converted into dopamine in the brain, which helps alleviate the symptoms of Parkinson’s disease.
Carbidopa is a dopa decarboxylase inhibitor that prevents the peripheral conversion of Levodopa into dopamine. This action increases the availability of Levodopa in the brain and reduces peripheral side effects, thereby enhancing therapeutic efficacy.
Dosage should be individualized based on patient response and clinical condition.
Therapy may be initiated with one tablet three or four times daily.
Gradual titration may be required to achieve optimal symptom control.
When higher Levodopa requirements are needed, higher-strength combinations may be substituted under medical supervision.
Experience with daily Carbidopa doses above 200 mg is limited.
Patients should follow the prescribing physician’s instructions carefully.
May cause symptomatic postural hypotension when used with antihypertensive medicines
Tricyclic antidepressants may rarely cause hypertension or dyskinesia
Iron salts (ferrous sulphate or gluconate) may reduce bioavailability
Dopamine-2 receptor antagonists, isoniazid, phenytoin, and papaverine may reduce therapeutic effects
Concomitant use with selegiline may cause severe orthostatic hypotension
Hypersensitivity to Levodopa or Carbidopa
Narrow-angle glaucoma
Suspicious undiagnosed skin lesions or a history of malignant melanoma
Common adverse effects include:
Dyskinesias such as choreiform or dystonic movements
Nausea
Most side effects are dose-related and may be reduced by dosage adjustment.
Use during pregnancy only if the potential benefit justifies the possible risk to the fetus
A decision should be made whether to discontinue breastfeeding or discontinue the medicine, considering the importance of therapy to the mother
Not recommended for drug-induced extrapyramidal reactions
Levodopa should be discontinued at least 12 hours before starting combination therapy
Monitor for depression, suicidal tendencies, and psychotic symptoms
Use cautiously in patients with cardiovascular, pulmonary, renal, hepatic, or endocrine disorders
Careful monitoring is advised in patients with glaucoma, arrhythmia, or a history of peptic ulcer disease
Safety and effectiveness in children and adolescents below 18 years have not been established
