Calcium-Channel Blockers
Larcadip 10 mg is indicated for:
Treatment of mild to moderate essential hypertension
Patients requiring long-term blood pressure control with once-daily dosing
It may be used alone or in combination with other antihypertensive agents such as beta-blockers, diuretics, or ACE inhibitors when monotherapy is insufficient.
Lercanidipine is a dihydropyridine calcium-channel blocker that inhibits the influx of calcium ions through slow L-type calcium channels in vascular smooth muscle during depolarization. This action results in relaxation of arterial smooth muscle and a reduction in peripheral vascular resistance, leading to decreased blood pressure.
Lercanidipine shows greater selectivity for vascular smooth muscle compared to cardiac muscle, which explains its potent antihypertensive effect without significant negative inotropic activity. Its high lipophilicity and membrane partition coefficient contribute to a prolonged and smooth antihypertensive action, allowing once-daily administration.
Adults and Elderly:
Recommended starting dose: 10 mg once daily
Taken orally at least 15 minutes before meals
Dose may be increased to 20 mg once daily depending on patient response
Maximum antihypertensive effect may take up to 2 weeks to become fully apparent
Renal or Hepatic Impairment:
Use with caution in mild to moderate impairment
Doses above 20 mg daily should be approached cautiously
Not recommended in severe renal or hepatic dysfunction
Children:
Use is not recommended in patients under 18 years due to lack of clinical data
Must be taken on an empty stomach
Avoid grapefruit juice, as it can increase plasma levels of Lercanidipine
Concomitant use should be avoided with:
Cyclosporin
Phenytoin, carbamazepine, rifampicin
Ketoconazole, itraconazole, ritonavir
Erythromycin, troleandomycin, midazolam
Caution is required when used with:
Astemizole, amiodarone, quinidine
Cimetidine, metoprolol, simvastatin
Larcadip 10 mg is contraindicated in patients with:
Left ventricular outflow tract obstruction
Untreated congestive heart failure
Unstable angina pectoris
Recent myocardial infarction (within 1 month)
Known hypersensitivity to dihydropyridines
Concomitant intake of grapefruit juice
Lercanidipine is generally well tolerated. Common side effects include:
Flushing
Peripheral edema
Headache
Dizziness
Asthenia
Less common side effects may include:
Fatigue
Gastrointestinal disturbances (nausea, dyspepsia, diarrhea)
Rash, somnolence, myalgia
Pregnancy Category C
Not recommended during pregnancy due to lack of clinical data
Should not be used by women of childbearing potential without effective contraception
Not recommended during lactation, as distribution into breast milk is likely
Use with caution in patients with ischemic heart disease
Special care in sick sinus syndrome or left ventricular dysfunction
Monitor blood pressure regularly
Overdose may cause excessive vasodilation, hypotension, and reflex tachycardia. Dialysis is unlikely to be effective due to high lipophilicity. Supportive treatment is recommended.
