4-Quinolone Antibiotic / Fluoroquinolone
Levoxin 500 mg is a third-generation, synthetic, chiral fluoroquinolone. Its antibacterial activity is achieved through inhibition of bacterial DNA gyrase and topoisomerase IV, enzymes crucial for DNA replication, transcription, and repair. By disrupting these processes, Levoxin effectively halts bacterial growth and induces cell death.
It exhibits in vitro activity against a wide spectrum of bacteria, including:
Gram-negative organisms such as Escherichia coli, Klebsiella spp., Pseudomonas aeruginosa
Gram-positive organisms such as Streptococcus pneumoniae, Staphylococcus aureus
Levoxin is rapidly absorbed after oral administration, achieving therapeutic concentrations in blood and tissues, including the lungs, urinary tract, and prostate.
Levoxin 500 mg is indicated for the treatment of adult patients (18 years and older) with:
Respiratory tract infections:
Acute bacterial sinusitis
Acute exacerbations of chronic bronchitis
Community-acquired pneumonia
Nosocomial pneumonia
Skin and soft tissue infections: Uncomplicated and complicated
Urinary tract infections (UTIs): Uncomplicated and complicated
Chronic bacterial prostatitis
Acute sinusitis: 500 mg once daily for 10–14 days
Chronic bronchitis exacerbations: 250–500 mg once daily for 7–10 days
Community-acquired pneumonia: 500 mg once or twice daily for 7–14 days
Nosocomial pneumonia: 750 mg once daily for 7–14 days
Skin and soft tissue infections: 500 mg once or twice daily for 7–14 days
Chronic prostatitis: 500 mg once daily for 28 days
Urinary tract infections: 250 mg daily for 7–10 days (3 days for uncomplicated cases)
Administration Tips: Take with a full glass of water. Maintain adequate hydration during therapy to reduce risk of crystalluria.
Hypersensitivity to Levoxin or other quinolone antibiotics
History of tendon disorders related to fluoroquinolone use
Levoxin 500 mg is generally well tolerated. Common side effects include:
Nausea, vomiting, diarrhea, abdominal pain, flatulence
Rarely: phototoxicity, tremors, anxiety, depression, confusion
Antacids, iron, and adsorbents: Reduce absorption of Levoxin
NSAIDs: May increase risk of CNS stimulation
Warfarin: May enhance anticoagulant effect, increasing bleeding risk
Adjust dosage in patients with renal impairment
Avoid excessive sun exposure due to potential phototoxicity
Monitor for tendon pain, especially in elderly patients or those on corticosteroids
FDA Pregnancy Category C: Use only if potential benefits outweigh risks
Not recommended during breastfeeding
